摘要
目的:观察厌食方联合针刺四缝穴治疗小儿虚实夹杂型厌食症的临床疗效及对血清神经肽Y(NPY)、瘦素(LEP)水平的影响。方法:选取2023年1月—2024年8月杭州师范大学附属萧山医院韩氏儿科门诊收治的厌食症患儿80例,采用随机数字表法分为对照组和治疗组,每组40例。对照组给予葡萄糖酸锌颗粒治疗,治疗组给予厌食方配合针刺四缝穴治疗。2组均治疗2个月。比较2组临床疗效、中医证候评分及血清NPY、LEP水平变化。结果:治疗组总有效率为87.5%(35/40),对照组为67.5%(27/40),2组比较,差异有统计学意义(P<0.05)。治疗后,2组食欲减退、食量减少、腹痛、嗳气腹胀、神疲、面色少华等证候评分均较治疗前降低(P<0.05),且治疗组上述各项证候(除腹痛外)评分均低于对照组(P<0.05);治疗组恶心呕吐、大便不调评分均较治疗前降低(P<0.05),且均低于对照组(P<0.05);而对照组恶心呕吐、大便不调评分治疗前后比较,差异无统计学意义(P>0.05)。治疗后,2组血清NPY、LEP水平均较治疗前升高(P<0.05),且治疗组NPY、LEP水平均高于对照组(P<0.05)。结论:厌食方配合针刺四缝穴治疗小儿厌食症疗效良好,可明显改善患儿中医证候,有效调节NPY、LEP水平,改善食欲。
Objective:To observe the clinical efficacy of Anorexia Formula combined with acupuncture at Sifeng(EX-UE10)points in treating pediatric anorexia with deficiency interweaved with excess syndrome and its effects on serum levels of neuropeptide Y(NPY)and leptin(LEP).Methods:A total of 80 children with anorexia treated at the Han's Pediatric Clinic of Xiaoshan Hospital Affiliated to Hangzhou Normal University from January 2023 to August 2024 were selected and divided into the control group and the treatment group according to the random number table method,with 40 cases in each group.The control group received Zinc Gluconate Granules,while the treatment group received Anorexia Formula combined with acupuncture at Sifeng(EX-UE10)points.Both groups were treated for two months.The clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,and changes in serum NPY and LEP levels were compared between the two groups.Results:The total effective rate in the treatment group was 87.5%(35/40),significantly higher than that in the control group(67.5%,27/40)(P<0.05).After treatment,scores for syndomes such as poor appetite,reduced food intake,abdominal pain,belching,abdominal distension,fatigue,and pale complexion decreased in both groups compared to before treatment(P<0.05),with the treatment group showing lower scores than the control group for all syndomes except abdominal pain(P<0.05).The scores for nausea,vomiting,and irregular bowel movements decreased in the treatment group compared to before treatment(P<0.05)and were lower than those in the control group(P<0.05),while there was no significant difference in these scores in the control group before and after treatment(P>0.05).After treatment,the serum NPY and LEP levels increased in both groups compared to before treatment(P<0.05),with the treatment group showing higher levels than the control group(P<0.05).Conclusion:Anorexia Formula combined with acupuncture at Sifeng(EX-UE10)points is effective in treating pediatric anorexia.It significantly improves TCM syndomes,regulates NPY and LEP levels,and enhances appetite.
