摘要
目的 建立多学科团队协作模式下的老年慢性心力衰竭(chronic heart failure,CHF)患者医院居家一体化服务体系,为老年CHF患者安宁疗护服务提供参考与支持。方法 2023年9月—2024年7月通过文献分析、访谈、专家小组会议初步确定老年CHF患者医院居家安宁疗护服务体系的整体架构并形成专家咨询调查问卷,向全国8个省市的护理教育、护理管理、临床护理、医疗领域18名专家经2轮咨询后确定服务体系的评价指标。结果 形成包括4个一级指标、8个二级指标、48个三级指标的老年CHF患者医院-居家安宁疗护服务体系。2轮专家函询的问卷回收率和有效率均为100%;2轮函询专家权威系数分别为0.84、0.87;总协调系数分别为0.127、0.076。第2轮函询各条目的变异系数为0~0.15,专家意见一致性良好。结论 本研究构建的服务体系具有较高的科学性、创新性和延展性,可为CHF患者安宁疗护一体化运行和发展提供参考。
Objective To establish an integrated service system for elderly patients with chronic heart failure(CHF)in hospital and at home under the multi-disciplinary team collaboration model,which can provide reference and support for the hospice care services for elderly patients with CHF.Methods From September 2023 to July 2024,through literature analysis,interviews and expert group meetings,the overall framework of the integrated service system for hospice care of elderly patients with CHF in hospital and at home were preliminarily determined and an expert consultation questionnaire was formed.After two rounds of consultation with 18 experts in nursing education,management,clinical nursing,and medical fields across 8 provinces and cities in China,the evaluation indicators of the service system were determined.Results A service system for elderly patients with CHF in hospital and at home were formed,including 4 first-level indicators,8 second-level indicators,and 48 third-level indicators.The questionnaire recovery rate and validity rate of the two rounds of expert consultation were both 100%.The expert authority coefficients of the two rounds of consultation were 0.84 and 0.87 respectively.The total coordination coefficients were 0.127 and 0.076 respectively.The coefficient of variation of each item in the second round of consultation was 0-0.15,and the consistency of expert opinions was good.Conclusion The service system constructed in this study is highly scientific,innovative,and extensible,and can provide reference for the integrated operation and development of hospice care for patients with CHF.
