摘要
文中从“凡土脏取决于胆”理论为切入点,以胆和腹泻型肠易激综合征(Diarrhea-predominant irritable bowel syndrome,IBS-D)发病的关系为着眼点,认为胆与IBS-D的关联性更强,并参与人体消化、免疫和精神等活动。当中医胆腑的生理功能失常时,会导致藏精汁助运化、主升发疏气机、主决断调情志、司勇怯御病邪和为枢纽通阴阳的职能失司,进而产生与IBS-D相关的临床症状。故而从“胆”阐释IBS-D的发病和治疗,以期为临床防治IBS-D提供新的思路。
Based on the theory of"earth zang organs depending on the gallbladder",this paper focuses on the relationship between the gallbladder and the pathogenesis of diarrhea-predominant irritable bowel syndrome(IBS-D).It is believed that the gallbladder has a stronger correlation with IBS-D and participates in human digestive,immune,and mental activities.When the physiological function of gallbladder organs in traditional Chinese medicine(TCM)is abnormal,the fllowing functions will be disturbed:storing essence,assisting transportation and transformation,governing ascent and dispersion,regulating qi movement,governing decision and regulating emotion,managing courage and timidity,resisting external invasion,and regulating Yin and Yang as the hub.All these lead to the clinical symptoms relevant to IBS-D.Therefore,the pathogenesis and treatment of IBS-D are explained from the perspective of"gallbladder",aiming to provide new ideas for clinical prevention and treatment of this disease.
