摘要
目的:分析血清白细胞介素-4(IL-4)、趋化因子C-C基元配体5(CCL5)、分泌性磷蛋白1(SPP1)、C反应蛋白(CRP)对肺炎支原体肺炎(MPP)患儿疾病严重程度的评估价值。方法:选取2023年1月—2023年12月某院100例MPP患儿为观察组,根据疾病严重程度分为重症组(n=32)和轻症组(n=68),并选取同期100例健康体检儿童为对照组。比较观察组与对照组及不同疾病严重程度患儿血清IL-4、CCL5、SPP1、CRP水平;采用Spearman系数分析血清IL-4、CCL5、SPP1、CRP水平与MPP患儿疾病严重程度的相关性;采用多因素Logistic回归方程分析MPP患儿疾病严重程度的危险因素;绘制ROC曲线评估血清IL-4、CCL5、SPP1、CRP对MPP患儿疾病严重程度的预测价值。结果:观察组血清IL-4、CCL5、SPP1、CRP水平高于对照组,重症组高于轻症组,差异均有统计学意义(P<0.05)。Spearman相关性分析结果显示,血清IL-4、CCL5、SPP1、CRP水平与MPP患儿疾病严重程度呈正相关关系(r>0,P均<0.001)。多因素Logistic回归分析显示,血清IL-4、CCL5、SPP1、CRP水平升高均是MPP患儿疾病严重程度的独立危险因素(OR>1,P均<0.05)。ROC曲线分析结果显示,血清IL-4、CCL5、SPP1、CRP水平预测MPP患儿病情严重程度AUC分别为0.854、0.846、0.907、0.872,联合检测的AUC为0.968,具有较高的预测价值。结论:血清IL-4、CCL5、SPP1、CRP水平升高均是MPP患儿疾病严重程度的独立危险因素,联合检测预测价值较高,可作为评估MPP患儿疾病严重程度的有效生物标志物。
Objective:To analyze the value of serum levels of interleukin-4(IL-4),Chemokine C-C motif ligand 5(CCL5),secretory phosphoprotein-1(SPP1)and C-reactive protein(CRP)in evaluating disease severity in children with mycoplasma pneumoniae pneumonia(MPP).Methods:100 children with MPP in a hospital from January 2023 to December 2023 were selected as an observation group,and 100 healthy children during the same period were recruited as a control group.The observation group was re-divided into a severe group(n=32)and a mild group(n=68)according to the severity of the disease.Serum levels of IL-4,CCL5,SPP1 and CRP were compared between the observation group and the control group,as well as among children with different disease severity.Spearman analysis was used to analyze the correlation of serum levels of IL-4,CCL5,SPP1 and CRP with the severity of MPP children.Multivariate Logistic regression analysis was adopted to analyze the risk factors for the disease severity.ROC curve was plotted to evaluate the predictive value of serum IL-4,CCL5,SPP1 and CRP levels for the disease severity.Results:Serum IL-4,CCL5,SPP1 and CRP levels in the observation group were higher than those in the control group(P<0.05),and were higher in the severe group than those in the mild group(P<0.05).Spearman correlation results showed that serum IL-4,CCL5,SPP1 and CRP levels were positively correlated with disease severity in MPP children(r>0,all P<0.001).Multivariate Logistic regression analysis showed that serum IL-4,CCL5,SPP1 and CRP levels were independent risk factors for the disease severity in MPP children(OR>1,all P<0.05).ROC curve analysis results indicated that serum IL-4,CCL5,SPP1 and CRP levels had high diagnostic value in evaluating the severity of MPP children,and their AUC was 0.854,0.846,0.907,and 0.872,respectively.The AUC of the combined detection was 0.968,which was better than each single index in prediction.Conclusion:Serum IL-4,CCL5,SPP1 and CRP levels can be used as effective biomarkers to evaluate the severity of MPP in children.
