摘要
目的探讨红景天对处于慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者血液中Toll样受体4(toll-like receptor 4,TLR4)/核因子κB(nuclear factor-κB,NF-κB)信号转导通路和炎症因子的影响。方法收集2023年9月~2024年8月我院门诊及住院的118例AECOPD患者,随机分为治疗组及对照组。对照组给予常规抗感染、雾化、平喘等治疗。治疗组在常规治疗基础上予以大株红景天静脉注射液滴注治疗10天。治疗前后应用聚合酶链式反应(polymerase chain reaction,PCR)法测定外周血单个核细胞中TLR4 mRNA和NF-κB mRNA的表达情况,采用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测血液中肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-8(interleukin-8,IL-8)水平。结果治疗后,治疗组TLR4mRNA和NF-κBmRNA的相对表达量低于对照组,差异有统计学意义(P<0.01);血液中TNF-α、IL-6、IL-8水平低于对照组,差异有统计学意义(P<0.01)。结论红景天可通过TLR4/NF-κB信号转导通路降低患者血液中TNF-α、IL-6、IL-8的表达水平,从而使AECOPD患者的体内炎症得到更好的控制。
Objective To investigate the effects of Hongjingtian on the Toll-like receptor 4(TLR4)/nuclear factor-κB(NF-κB)signaling pathway and inflammatory cytokine levels in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 118 AECOPD patients admitted to our hospital from Sep 2023 to Aug 2024 were randomly divided into treatment group and control group.The control group was given conventional treatments such as anti infection,nebulization,and asthma relief,while the treatment group received intravenous infusion of Hongjingtian injection for 10 days on the basis of conventional treatment.Before and after treatment,polymerase chain reaction(PCR)was used to determine the expression of TLR4 mRNA and NF-κB mRNA in peripheral blood mononuclear cells,and enzyme-linked immunosorbent assay(ELISA)was used to test the blood levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-8(IL-8).Results After treatment,the relative expression levels of TLR4 mRNA and NF-κB mRNA in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.01).The levels of TNF-α,IL-6,and IL-8 in the blood were lower than those in the control group,and the differences were statistically significant(P<0.01).Conclusion Hongjingtian can reduce the expression levels of TNF-α,IL-6,and IL-8 in the blood of patients through the TLR4/NF-κB signaling pathway,thereby achieving better control of inflammation in patients with AECOPD.
