摘要
目的:分析中国老年人抑郁症状的潜在类别及影响因素。方法:使用2020年中国健康与养老追踪调查数据,采用潜在剖面分析对中国老年人抑郁症状进行分类,采用无序多分类Logistic回归分析中国老年人抑郁症状的影响因素。结果:中国老年人抑郁症状可分为无抑郁(73.89%)、轻度抑郁-高希望(6.95%)、中度抑郁-低希望(10.83%)、重度抑郁-高孤独(8.33%)4个类别。无序多分类Logistic回归分析结果表明,健康状况不好、夜间睡眠时间0~<5 h的老年人更易出现抑郁症状(P<0.05)。其中男性更不容易出现轻度抑郁-高希望(OR=0.43,95%CI为0.34~0.55)、重度抑郁-高孤独(OR=0.52,95%CI为0.42~0.65);居住地为农村、文化程度为小学及以下、不上网者更易出现中度抑郁-低希望、重度抑郁-高孤独(P<0.05);文化程度为初中者更易出现中度抑郁-低希望(OR=2.04,95%CI为1.27~3.28);与伴侣居住者更不容易出现轻度抑郁-高希望(OR=0.24,95%CI为0.60~0.94)。结论:中国老年人抑郁症状存在异质性,应根据不同抑郁类别,采取针对性措施进行干预。
Aim:To analyze the potential profile categories and influencing factors of depressive symptoms in the elderly in China.Methods:Based on the data of China Health and Retirement Longitudinal Study in 2020,the depression symptoms of the elderly were classified by potential profile analysis,and the influencing factors of each category were analyzed by disordered multinomial Logistic regression.Results:The depressive symptoms was divided into 4 categories:no depression(73.89%),mild depression-high hope(6.95%),moderate depression-low hope(10.83%),and severe depression-high loneliness(8.33%).Disordered multinomial Logistic regression analysis showed that the elderly with poor self-rated health and nighttime sleep time of 0-<5 hours were more prone to depressive symptoms(P<0.05).Men were less likely to develop mild depression-high hope(OR=0.43,95%CI was 0.34-0.55)and severe depression-high loneliness(OR=0.52,95%CI was 0.42-0.65);living in rural areas,education level of primary school and below,not surfing the Internet were more prone to moderate depression-low hope,severe depression-high loneliness(P<0.05);the education level of junior high school was more likely to develop moderate depression-low hope(OR=2.04,95%CI was 1.27-3.28);living with a partner was less likely to have mild depression-high hope(OR=0.24,95%CI was 0.60-0.94).Conclusion:The depressive symptoms of the elderly in China are heterogeneous,and targeted measures should be taken for intervention according to different categories.
