摘要
目的系统评价阿立哌唑对比奥氮平治疗老年痴呆伴发的精神行为症状(behavioral and psychological symptoms of dementia,BPSD)的疗效。方法通过计算机检索万方、维普、知网、中国生物医学文献数据库、PubMed、Embase、Cochrane Library数据库中阿立哌唑对比奥氮平治疗老年痴呆精神行为症状的随机对照试验,检索时间从建库至2022年8月,使用Review Manager 5.3软件进行Meta分析。结果最后共有6篇文献被纳入研究,共计收集466例BPSD病例,试验组236例(阿立哌唑组),对照组230例(奥氮平组)。Meta分析显示,试验组总有效率(OR=1.54,95%CI 0.92~2.59)、阿尔茨海默病病理行为评定量表(BEHAVE-AD评分)(4周:MD=0.21,95%CI-0.35~0.76;8周:MD=0.12,95%CI-0.37~0.62)评分比较,差异均无统计学意义(P>0.05)。阿立哌唑组嗜睡(OR=0.33,95%CI 0.18~0.60)、体质量增加(OR=0.25,95%CI 0.11~0.62)的发生率低于对照组,差异有统计学意义(P<0.05)。口干、头晕、便秘、锥体外系反应发生率比较,差异均无统计学意义(P>0.05)。结论阿立哌唑治疗老年痴呆精神行为症状疗效不低于奥氮平,嗜睡和体质量增加发生率低于奥氮平。
Objective To systematically evaluate the efficacy of aripiprazole compared to olanzapine in the treatment of behavioral and psychological symptoms of dementia(BPSD).Methods Randomized controlled trials of aripiprazole compared to olanzapine in the treatment of mental and behavioral symptoms of elderly dementia were searched through computer databases such as Wanfang,CNKI,VIP database,Chinese Biomedical Literature database,PubMed,Embase and Cochrane Library.The search period was from database establishment to August 2022,and Meta-analysis was performed using review manager 5.3 software.Results The results included 6 articles,a total of 466 cases of BPSD were collected,with 236 in the experimental group(aripiprazole group)and 230 in the control group(olanzapine group).Meta analysis indicated that the total effective rate(OR=1.54,95%CI 0.92-2.59)and BEHAVE-AD(the fourth week:MD=0.21,95%CI-0.35-0.76;eighth week:MD=0.12,95%CI-0.37-0.62)score of the experimental group were not statistically significant(P>0.05).The incidence of drowsiness(OR=0.33,95%CI0.18-0.60)and weight gain(OR=0.25,95%CI0.11-0.62)in the aripiprazole group was lower than that in the control group,with statistical significance(P<0.05).There was no any statistically significant difference in the incidence of dry mouth,dizziness,constipation,and extrapyramidal reactions(P>0.05).Conclusion Aripiprazole is not inferior to olanzapine in the treatment of mental and behavioral symptoms in elderly dementia,and the incidence of drowsiness and weight gain are lower than olanzapine.
