超声引导下不同髂筋膜间隙阻滞方式对全髋关节置换术患者的影响研究

Study on the Effect of Different Modes of Fascia Iliaca Compartment Block under Ultrasound Guidance on Patients Undergoing Total Hip Arthroplasty

摘要比较超声引导下腹股沟韧带上髂筋膜间隙阻滞(superior iliofascial space block of inguinal ligament,S-FICB)和经腹股沟韧带髂筋膜间隙阻滞(inferior fascia iliaca compartment block,I-FICB)对全髋置换术患者的影响。选取2023年1月-2024年4月医院收治的72例需行全髋关节置换术的患者为研究对象,随机将其分为S组和I组,每组各36例。S组行S-FICB,I组则行I-FICB。比较两组患者的相关指标结果显示,S组阻滞后15 min和30 min的内收肌群肌力低于I组和阻滞前(P<0.05),S组术中和术后PCIA舒芬太尼用量少于I组(P<0.05);两组手术时间、术中出血量、拔管时间、局部麻醉药物中毒发生率和神经损伤发生率比较,无统计学差异(P>0.05)。研究发现,S-FICB可降低大腿内收肌群肌力,有效阻滞闭孔神经,减轻全髋关节置换术患者围手术期疼痛。 To compare the inﬂuence of the superior iliofascial space block of inguinal ligament(S-FICB)and the inferior fascia iliaca compartment block(I-FICB)under ultrasound guidance on patients undergoing total hip arthroplasty.A total of 72 patients who needed to undergo total hip arthroplasty and were admitted to the hospital from January 2023 to April 2024 were selected as the research objects.They were randomly divided into group S and group I,with 36 cases in each group.Group S received S-FICB,and group I received I-FICB.The relevant indicators of the two groups were compared.The results showed that the muscle strength of the adductor muscles in group S 15 minutes and 30 minutes after the block was lower than that in group I and before the block(P<0.05),the dosage of sufentanil for PCIA during the operation and after the operation in group S was less than that in group I(P<0.05);there were no signiﬁcant diﬀerences in the operation time,the intraoperative blood loss,the extubation time,the incidence of local anesthetic poisoning,and the incidence of nerve injury between the two groups(P>0.05).The study found that S-FICB can reduce the muscle strength of the adductor muscles of the thigh,eﬀectively block the obturator nerve,and relieve the perioperative pain of patients undergoing total hip arthroplasty.
