摘要
目的探讨两种入路髂筋膜间隙阻滞(FICB)在全髋关节置换术(THA)中的效果。方法将福州市第二总医院2021年6月至2023年5月202例THA患者分为A组和B组(每组101例)。A组以腹股沟韧带上入路,B组以腹股沟韧带下入路。对比两组的术中生命体征和主观疼痛程度及阻滞效果。结果在术中心率、平均动脉压及脑电双频指数,A组与B组无明显差异(P>0.05)。在术后各时间点(T_(1)~T_(4)),两组患者静息和被动活动的VAS均无明显差异(P>0.05)。A组在T_(1),T_(2)及T_(3)时的股外侧皮神经(LFCN)及闭孔神经(ON)的阻滞成功率均比B组高(P<0.05)。结论腹股沟韧带上和下两种入路行FICB在THA中对血流动力学影响较小,前者的LFCN及ON阻滞效果优于后者。
Objective To study the anesthetic effects of two approaches of fascia iliaca compartment block(FICB)in total hip arthroplasty(THA).Methods Two hundred and two patients who underwent elective THA were selected from the Second General Hospital of Fuzhou from June 2021 to May 2023,and were divided into group A and group B(n=101).Patients in group A received the FICB via the superior approach of the inguinal ligament,while patients in group B received the FICB via the inferior approach of the inguinal ligament.The intraoperative heart rate,mean arterial pressure,and bispectral index of the two groups of patients;postoperative resting and passive movement visual analog scale(VAS)scores;and postoperative block efficacy were observed.Results There were no significant differences in intraoperative heart rate,mean arterial pressure and bispectral index between the two groups(P>0.05).At each time point after surgery,there were no significant differences in resting VAS and passive VAS between the two groups(P>0.05).The success rates of lateral femoral cutaneous nerve and obturator nerve in group A were higher than those in group B(P<0.05).Conclusions Both approaches can be used for fascia iliaca compartment block in total hip arthroplasty,and the effect of the former LFCN and ON block is better than the latter.
