内镜泪囊鼻腔吻合术下加用扩展性骨窗及高位造口对慢性泪囊炎的手术效果及血清血管内皮生长因子、降钙素原水平的影响

摘要目的对慢性泪囊炎患者采用内镜泪囊鼻腔吻合术下加用扩展性骨窗及高位造口治疗,探讨其手术效果及对血清血管内皮生长因子(VEGF)、降钙素原(PCT)水平的影响。方法选取2021年1月至2023年12月漳浦县医院收治的慢性泪囊炎患者70例79眼,将其资料进行回顾性分析,根据随机数字表法分为对照组(33例39眼,单纯造口术)和观察组(37例40眼,内镜泪囊鼻腔吻合术下加用扩展性骨窗及高位造口)。比较两组的血清VEGF、PCT水平、手术指标及并发症情况。结果相比对照组,术后观察组的VEGF、PCT水平均降低(P<0.05)。观察组的手术时间、术中出血量及住院时间均少于对照组(P<0.05)。观察组的并发症发生率(7.5%)低于对照组(25.64%)(P<0.05)。结论采用内镜泪囊鼻腔吻合术下加用扩展性骨窗及高位造口治疗慢性泪囊炎,能有效改善患者的血清VEGF、PCT水平,缩短治疗时间,降低并发症,临床手术效果显著。 Objective To investigate the effect of endoscopic dacryocystitis combined with extended bone window and high ostomy on the level of Serum vascular endothelial growth factor and procalcitonin in patients with chronic dacryocystitis.Methods A total of 79 eyes of 70 patients with chronic dacryocystitis treated in Zhangpu County Hospital from January 2021 to December 2023 were retrospectively analyzed,and their data were random number table method divided into control group(33 cases,39 eyes,simple neostomy)and observation group(37 cases,40 eyes,endoscopic dacryocystisinostomy with extended bone window and high stomy).The levels of serum VEGF,PCT,operative indexes and complications were compared between the two groups.Results Compared with the control group,the levels of VEGF and PCT in the observation group were significantly decreased(P<0.05).The operative time,intraoperative blood loss and hospital stay in the observation group were lower than those in the control group(P<0.05).The complication rate of observation group(7.5%)was lower than that of control group(25.64%)(P<0.05).Conclusions Endoscopic dacryocystosinostomy combined with extended bone window and high ostomy in the treatment of chronic dacryocystitis can effectively improve the levels of serum VEGF and PCT,shorten the treatment time,reduce complications,and achieve significant clinical results.
