摘要
目的研究广西南宁市首届名中医易蕾治疗子宫内膜异位症的遣方及用药规律,并探讨其诊治该症的学术思想及经验。方法筛选2020年3月-2023年9月由易蕾教授诊治子宫内膜异位症的中药处方,建立数据库,并应用中医传承计算平台V3.0对中药数据进行频次统计、功效统计、性味归经统计、关联规则分析、k-means聚类分析,并探究核心处方。结果纳入中药处方共978例,涉及269味中药,累计使用频次17085次,包含赤芍、当归、五灵脂、茯苓等17味高频中药(使用频次≥300次);中药功效以补虚类(26.62%)、活血化瘀类(22.53%)、理气类(9.42%)、清热类(7.97%)为主;药性以温、平为主,药味主要苦味药,甘味药,辛味药;药味归经以肝经、脾经、心经为主;基于关联规则挖掘“赤芍,五灵脂”“赤芍,益母草”“赤芍,柴胡”等核心对药组合17组,川芎→当归、桃仁→赤芍、桃仁→五灵脂等21条药对;基于k-means算法结合回归模型聚类挖掘出5类核心组合。结论易蕾教授治疗该症以活性化瘀为核心,辨证论治,以此为基础针对寒凝血瘀证、气虚血瘀证、气滞血瘀证、湿热瘀阻证等不同证型,酌情配伍温里药、补益药、理气药、清热凉血药。
Objective To study the prescription and medication rules of the first famous Chinese medicine YI Lei in Nanning,Guangxi in the treatment of endometriosis,and to explore its academic thoughts and experience in the diagnosis and treatment of this disease.Methods The traditional Chinese medicine prescriptions for endometriosis diagnosed and treated by Professor YI Lei from March 2020 to September 2023 were screened,and a database was established.The traditional Chinese medicine inheritance calculation platform V3.0 was used to perform frequency statistics,efficacy statistics,taste and meridian tropism statistics,association rule analysis,k-means clustering analysis on the traditional Chinese medicine data,and the core prescriptions were explored.Results A total of 978 traditional Chinese medicine prescriptions were included,involving 269 Chinese medicines,with a cumulative frequency of 17085 times,including 17 high-frequency Chinese medicines such as Chishao,Danggui,Wulingzhi,Fuling(frequency of use≥300 times);the efficacy of traditional Chinese medicine was mainly tonic(26.62%),blood-activating and stasis-resolving(22.53%),qi-regulating(9.42%)and heat-clearing(7.97%).The medicinal properties were mainly warm and flat,and the medicinal taste was mainly bitter,sweet and pungent.The meridian tropism of medicine was mainly liver meridian,spleen meridian and heart meridian.Based on the association rules,17 groups of core drug combinations such as"Chishao,Wulingzhi""Chishao,Yimucao""Chishao,Chaihu"were excavated,and 21 drug pairs such as Chuanxiong→Danggui,Taoren→Chishao,Chishao→Wulingzhi were excavated.Based on k-means algorithm combined with regression model clustering,5 core combinations were mined.Conclusion Professor YI Lei treats the disease with active stasis as the core.Through syndrome differentiation and treatment,patients with different syndromes such as cold coagulation and blood stasis syndrome,qi deficiency and blood stasis syndrome,qi stagnation and blood stasis syndrome,and damp heat and blood stasis syndrome are treated with warming drugs,tonic drugs,qi-regulating drugs,and heat-clearing and blood-cooling drugs.
