基于流程再造理论的胸痛中心模式下标准化急性心肌梗死急救护理流程的构建

摘要 目的:基于流程再造理论构建胸痛中心模式下标准化急性心肌梗死(AMI)急救护理流程。方法:以流程再造理论为基础,通过现场实地追踪调查的方法对原AMI急救流程进行复盘;结合超时节点原因分析及半结构式访谈的结果对原流程进行诊断分析;对关键环节制订改进措施后对流程进行重构;通过2轮专家函询对流程内容进行持续改进,完成胸痛中心模式下标准化AMI急救护理流程的构建。结果:构建了包括3个层次的胸痛中心模式下标准化AMI急救护理流程,其中一级流程6条,二级流程14条,三级流程28条。2轮专家函询的问卷回收率均为100%,专家权威系数均为0.875,第2轮肯德尔和谐系数高于第1轮。结论:该研究基于流程再造理论构建的胸痛中心模式下标准化AMI急救护理流程具有较强的科学性、可靠性和实用性,对提高AMI急救护理水平和效率具有重要意义,也为胸痛专科护士的培养提供了新的思路。 Objective:To construct a standardized emergency care process for acute myocardial infarction(AMI)under the chest pain center model based on process reengineering theory.Methods:Based on process reengineering theory,the original AMI emergency care process was reviewed through on⁃site tracking and investigation.Diagnostic analysis of the original process was conducted by combining timeout node cause analysis and semi⁃structured interviews.Key steps were identified,and improvement measures were formulated to reconstruct the process.The process contents were continuously refined through two rounds of expert consultation,ultimately completing the construction of a standardized AMI emergency care process under the chest pain center model.Results:The standardized AMI emergency care process under the chest pain center model was constructed with 3 levels,6 primary processes,14 secondary processes,and 28 tertiary processes.The questionnaire response rate for both rounds of expert consultation was 100%,with an expert authority coefficient of 0.875.The Kendall's coefficient of concordance was higher in the second round compared to the first.Conclusions:The standardized AMI emergency care process constructed in this study,based on process reengineering theory,demonstrates strong scientific rigor,reliability,and practicality.It holds significant importance for improving the quality and efficiency of AMI emergency care and provides new insights for the training of specialized chest pain nurses.
