摘要
目的:探讨入院时CT灌注成像参数与血清泛连接蛋白1、N末端脑钠肽前体(N-terminal precursor brain natriureticpeptide,NT-proBNP)联合检测对急性缺血性脑卒中(Acuteischemic stroke,AIS)患者不良预后的预测价值。方法:回顾性选取2023年1月至2023年12月郑州市第十五人民医院就诊的248例AIS患者作为研究组,另选取248例同期健康体检者作为对照组,比较两组及不同严重程度AIS患者入院时CT灌注成像参数及血清泛连接蛋白1、NT-proBNP水平,分析上述指标与AIS病情严重程度相关性;联合检测对AIS患者不良预后的预测价值。结果:研究组入院时脑血流量、脑血容量水平低于对照组,血流平均通过时间(Meantransittime,MTT)、血清泛连接蛋白1、NT-proBNP水平高于对照组(P<0.05);不同严重程度AIS入院时脑血流量、脑血容量比较:轻度>中度>重度,MTT、血清泛连接蛋白1、NT-proBNP水平比较:轻度<中度<重度(P<0.05);脑血流量、脑血容量与AIS病情严重程度呈负相关,MTT、血清泛连接蛋白1、NT-proBNP水平与病情严重程度呈正相关(P<0.05);入院时CT灌注成像参数与血清泛连接蛋白1、NT-proBNP水平联合检测对AIS患者不良预后预测AUC大于上述指标单独检测。结论:CT灌注成像参数及血清泛连接蛋白1、NT-proBNP水平与AIS患者严重程度存在显著相关性,上述指标联合检测对AIS患者不良预后具有较高的预测效能。
Objective:To explore the predictive value of CT perfusion imaging parameters and serum levels of Pannexin^(-1) and N-terminal precursor brain natriuretic peptide(NT-proBNP)at admission combined detection for poor prognosis of acute ischemic stroke(AIS).Methods:A total of 248 AIS patients treated in 15th People's Hospital of Zhengzhou from Jan.2023 to Dec.2023 were retrospectively selected as the study group,and 248 healthy subjects during the same period were selected as the control group.CT perfusion imaging parameters,serum levels of Pannexin^(-1) and NT-proBNP at admission were compared between two groups,among different severity of AIS patients;and the correlation of the above indicators with the severity of of AIS patients was analyzed,the predictive value of the combined detection of the above indicators for the poor prognosis of AIS patients was analyzed.Results:Cerebral blood flow,cerebral blood volume in the study group in study group were lower than those in control group,mean transit time(MTT),serum levels of Pannexin^(-1) and NT-proBNP were higher than those in control group(P<0.05);comparison of cerebral blood flow,cerebral blood volume of AIS patients with different severity at admission were mild>moderate>severe;comparison of MTT,Pannexin^(-1),NT-proBNP were mild<moderate<severe(P<0.05);cerebral blood flow,cerebral blood volume were negatively correlated with the severity of AIS patients,while MTT,serum levels of Pannexin^(-1) and NT-proBNP were positively correlated with the severity of AIS patients(P<0.05);the AUC of cerebral blood flow,cerebral blood volume,MTT combined with serum levels of Pannexin^(-1) and NT-proBNP at admission for predicting poor prognosis in AIS patients was higher than separately detected by the above indicators.Conclusion:CT perfusion imaging parameters and serum levels of Pannexin1 and NT-proBNP are significantly correlated with the severity of AIS patients.The combined detection of these indicators has high predictive efficacy for the poor prognosis of AIS patients.
