摘要
目的探究连续胸壁振动与同步有氧训练对慢性阻塞性肺疾病(COPD)患者呼吸困难、肺功能及运动能力的有效性。方法2021年5月-2024年2月前瞻性纳入南阳市中心医院处于稳定期的COPD患者112例为研究对象,采用随机数字表法,将患者分为研究组和对照组,每组各56例,对照组实施常规护理与同步有氧训练,研究组则在此基础上增加连续胸壁振动治疗。干预3个月后记录并比较两组呼吸困难程度、肺功能参数、运动能力(6MWT步行距离)、生活质量(SGRQ-C)的差异。结果两组干预3个月后呼吸困难程度均较干预前降低(Z=6.999、5.340,P<0.05),且研究组呼吸困难程度低于对照组(Z=2.889,P<0.05)。重复测量方差分析显示,与干预前相比,两组在干预1个月及3个月后的第一秒用力呼气容积占预计值的百分比(FEV1),用力肺活量(FVC)与每分钟最大通气量占预计值的百分比(MVV)均显著提升(P<0.05),且研究组提升幅度更大(P<0.05)。干预3个月6 min步行试验(6MWT)的FEV1、FVC、MVV,6MWT步行距离高于干预后1个月(P<0.05)。干预3个月后,两组SGRQ-C量表各项评分(影响、活动、症状)较干预前均显著降低(P<0.05),且研究组降低幅度更大(P<0.05)。结论连续胸壁振动与同步有氧训练应用于COPD患者具有较好临床价值,可有效改善患者呼吸困难程度,促进肺功能恢复,提高运动功能,提升生活质量。
Objective To investigate the effectiveness of continuous chest wall vibration and synchronised aerobic training on dyspnoea,lung function and exercise capacity in patients with chronic obstructive pulmonary disease(COPD).Methods Between May 2021 and February 2024,112 patients with COPD in the stable stage were prospectively included in the study,and the patients were divided into a study group and a control group,with 56 cases in each group by using the method of randomized numerical table,and the control group implemented the routine care and synchronized aerobic training,while the study group added continuous chest wall vibration treatment on this basis.After 3 months of intervention,the differences in dyspnoea,pulmonary function parameters,exercise capacity(6MWT walking distance),and quality of life(SGRQ-C)between the two groups were recorded and compared.Results Dyspnoea was lower in both groups after 3 months of intervention compared with pre-intervention(Z=6.999,5.340,P<0.05)and was lower in the study group than that in the control group(Z=2.889,P<0.05).Repeated-measures ANOVA showed that first-second force expiratory volume as a percentage of predicted value(FEV1),forceful lung capacity(FVC)and maximum ventilation per minute as a percentage of predicted value(MVV)were significantly improved in both groups compared with the pre-intervention period at 1 month and 3 months after the intervention(P<0.05),and the improvement was greater in the study group(P<0.05).The 6MWT walking distance was significantly elevated in both groups after 1 and 3 months of intervention(P<0.05),and the elevation was greater in the study group(P<0.05).After 3 months of intervention,all SGRQ-C scale scores(affect,activity,and symptoms)were significantly lower in both groups compared to pre-intervention(P<0.05),and the reduction was greater in the study group(P<0.05).Conclusion Continuous chest wall vibration and synchronous aerobic training have good clinical value in patients with COPD,which can effectively improve the degree of dyspnea,promote the recovery of lung function,improve motor function and improve the quality of life.
