纤支镜肺泡灌洗联合头孢哌酮钠舒巴坦钠治疗支气管扩张症合并感染患者的效果

摘要目的:观察纤支镜肺泡灌洗联合头孢哌酮钠舒巴坦钠治疗支气管扩张症合并感染患者的效果。方法:回顾性分析2022年2月至2023年2月该院收治的110例支气管扩张症合并感染患者的临床资料,按治疗方案不同分为观察组和对照组各55例。对照组采用注射用头孢哌酮钠舒巴坦钠治疗,观察组在对照组基础上联合纤支镜肺泡灌洗,比较两组住院时间、血气分析指标[动脉血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]水平、炎性指标[白细胞计数(WBC)、中性粒细胞(NE)、C反应蛋白(CRP)]水平、血清细胞因子[基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶抑制剂-1(TIMP-1)]水平和不良反应发生率。结果:观察组住院时间明显短于对照组,差异有统计学意义(P<0.05);治疗后,观察组SaO_(2)、PaO_(2)、TIMP-1水平高于对照组,PaCO_(2)、WBC、NE、CRP、MMP-9水平低于对照组,差异均有统计学意义(P<0.05);治疗期间,两组均无明显不良反应发生。结论:纤支镜肺泡灌洗联合头孢哌酮钠舒巴坦钠治疗支气管扩张症合并感染患者可缩短住院时间,改善血气分析指标和血清细胞因子水平,以及降低炎性指标水平,效果优于单纯头孢哌酮钠舒巴坦钠治疗。 Objective:To observe effects of bronchofiberscope alveolar lavage combined with Cefoperazone sodium and Sulbactam sodium in treatment of patients with bronchiectasis complicated with infection.Methods:The clinical data of 110 patients with bronchiectasis complicated with infection admitted to this hospital from February 2022 to February 2023 were retrospectively analyzed.According to different treatment options,they were divided into observation group and control group,55 cases in each group.The control group was treated with Cefoperazone sodium and Sulbactam sodium for injection,while the observation group was combined with bronchofiberscope alveolar lavage on the basis of that of the control group.The hospitalization time,the levels of blood gas analysis indexes[arterial oxygen saturation (SaO_(2)),arterial oxygen partial pressure (PaO_(2)),arterial carbon dioxide partial pressure (PaCO_(2))],inflammatory indexes[white blood cell count (WBC),neutrophil (NE),C-reactive protein (CRP)]and serum cytokines[matrix metalloproteinase-9 (MMP-9),matrix metalloproteinase inhibitor-1 (TIMP-1)],and the incidence of adverse reactions were compared between the two groups.Results:The hospitalization time of the observation group was significantly shorter than that of the control group,and the difference was statistically significant (P<0.05).After treatment,the levels of SaO_(2),PaO_(2) and TIMP-1 in the observation group were higher than those in the control group,the levels of PaCO_(2),WBC,NE,CRP and MMP-9 were lower than those in the control group,and the differences were statistically significant (P<0.05).During the treatment,there were no obvious adverse reactions in the two groups.Conclusions:Bronchofiberscope alveolar lavage combined with Cefoperazone sodium and Sulbactam sodium in the treatment of the patients with bronchiectasis complicated with infection can shorten the hospitalization time,improve the levels of blood gas analysis indexes and serum cytokines,and reduce the levels of inflammatory indexes.Moreover,it is superior to simple Cefoperazone sodium and Sulbactam sodium treatment.
