摘要
目的:探讨心肾不交证不寐患者应用六味地黄汤合黄连阿胶汤加减治疗的临床效果。方法:选择2022年3月—2024年2月新疆生产建设兵团第三师总医院收治的心肾不交证不寐患者80例,以随机数字表法分为对照组(n=40,给予艾司唑仑片治疗)、观察组(n=40,给予艾司唑仑片联合六味地黄汤合黄连阿胶汤加减治疗)。评估两组睡眠改善效率、中医症候疗效、睡眠质量、焦虑和抑郁情绪、中医症候积分、神经递质指标水平及副作用。结果:观察组睡眠改善总有效率、中医症候总有效率及治疗后γ氨基丁酸(GABA)、褪黑素(MT)、5-羟色胺(5-HT)均较对照组高,差异均有统计学意义(P<0.05)。观察组治疗后匹兹堡睡眠质量指数(PSQI)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分及主症、次症积分、多巴胺(DA)水平均较对照组低,差异均有统计学意义(P<0.05)。两组副作用发生率比较,差异无统计学意义(P>0.05)。结论:心肾不交证不寐患者应用六味地黄汤合黄连阿胶汤加减治疗的效果佳,可改善患者临床症状,提高睡眠质量,调节神经递质,且安全性高。
Objective:To explore the clinical effect of modified Liuwei Dihuang Decoction and Huanglian Ejiao Decoction on patients with insomnia with syndrome of incoordination between heart and kidney.Method:A total of 80 patients with insomnia with syndrome of incoordination between heart and kidney admitted to General Hospital of the Third Division of Xinjiang Production and Construction Corps from March 2022 to February 2024 were divided into control group(n=40,treated with Estazolam Tablets)and observation group(n=40,treated with Estazolam Tablets combined with modified Liuwei Dihuang Decoction and Huanglian Ejiao Decoction)by random number table method.The sleep improvement efficiency,TCM syndrome efficacy,sleep quality,anxiety and depression,TCM syndromes scores,neurotransmitters indexes levels and side effects in both groups were evaluated.Result:The total effective rate of sleep improvement,total effective rate of TCM syndrome andγ-aminobutyric acid(GABA),melatonin(MT)and 5-hydroxytryptamine(5-HT)after treatment in observation group were higher than those in control group,the differences were statistically significant(P<0.05).The scores of Pittsburgh sleep quality index(PSQI),Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA),the scores of major symptoms and secondary symptoms,and dopamine(DA)levels in the observation group after treatment were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of side effects between the two groups(P>0.05).Conclusion:The application of modified Liuwei Dihuang Decoction and Huanglian Ejiao Decoction in the treatment of patients with insomnia with syndrome of incoordination between heart and kidney has a good effect,which can improve patients'clinical symptoms,improve sleep quality and regulate neurotransmitters,and has high safety.
