摘要
糖尿病性勃起功能障碍(ED)是糖尿病常见并发症之一,属难治性勃起功能障碍(RED),临床治疗策略尚需探讨。基于“络病理论”并结合阴茎海绵体特有解剖结构,提出糖尿病性ED等RED应归属为“阴茎络病”的观点,继而从病位、病因、病理三位一体探讨糖尿病性ED病机及治法。“阴茎络脉”为糖尿病性ED之微观病位,“内热伏体”状态是糖尿病性ED发生发展的特异性病因,阴茎络脉微癥积则是糖尿病性ED的关键病理变化,治疗重在益气活血、通络消癥,兼除伏热之邪。
Diabetic erectile dysfunction(ED)is one of the common complications of diabetes and is classified as refractory erectile dysfunction(RED),for which clinical treatment strategies still need to be explored.Based on the"collateral disease theory"and the unique anatomical structure of the penile corpus cavernosum,it was proposed that RED,including diabetic ED,should be classified as"penis collateral disease".From this perspective,the pathogenesis and treatment methods of diabetic ED were discussed through a trinity approach,considering disease location,etiology,and pathology.The"penile collateral vessels"represent the micro-location of diabetic ED.The"internal heat"condition is a specific cause of the onset and progression of diabetic ED,and micro-lumps in the penile collateral vessels are the key pathological changes in diabetic ED.Treatment should focus on replenishing qi and activating blood,dredging collaterals,dispelling micro-lumps,and clearing the internal heat pathogen.
