摘要
目的探讨益气活血补肾方对脾肾气虚血瘀型膜性肾病(MN)的临床疗效及氧化应激的影响。方法选取2023年3—7月于潍坊医学院附属医院经肾活检病理或磷脂酶A2受体抗体(Anti-PLA-2R)阳性的MN患者80例,随机抽签法分为观察组(40例)与对照组(40例),其中观察组脱落1例、对照组脱落2例。所有患者予环孢素A+强的松的标准治疗,观察组加服益气活血补肾方。治疗6个月后,观察两组患者中医症状积分、临床试验指标及血清超氧化物歧化酶(SOD)活性、丙二醛(MDA)、细胞因子信号转导抑制因子(SOCS)1、SOCS 3的含量等。结果治疗后,观察组中医症状积分、胆固醇(CHOL)、甘油三酯(TG)、Anti-PLA-2R、24 h尿蛋白定量(UTP)、尿足细胞顶端膜蛋白肌酐比值(PCX/Cr)均低于治疗前及对照组,且血清白蛋白(ALB)高于治疗前及对照组,差异有统计学意义(P<0.05,P<0.01);观察组SOD高于治疗前及对照组,且MDA、SOCS1、SOCS3均低于治疗前及对照组,差异有统计学意义(P<0.05,P<0.01)。结论益气活血补肾方可改善MN患者氧化应激状态,提高尿蛋白缓解率,改善临床疗效。
Objective This study aims to explore the effects of Yiqi Huoxue Bushen Prescription on the clinical efficacy and oxidative stress of membranous nephropathy featured by spleen and kidney qi deficiency and blood stasis.Methods A total of 80 patients who were positive in kidney biopsy or phospholipase A2 receptor antibody(Anti-PLA-2R)from March to July in the affiliated hospital of Weifang Medical University were selected and divided randomly into the observation group(40 patients)and the control group(40 patients),with one case in the observation group and two cases in the control group dropped out,respectively.Standard treatment composed of cyclosporine A and prednisone was given to all of the above patients,with Yiqi Huoxue Bushen Prescription additionally provided for the control group.After 6 months of treatment,the TCM symptom scores,clinical indexes and serum superoxide dismutase(SOD)activity and malondialdehyde(MDA)content were observed in both groups.Results After treatment,the TCM symptom score,cholesterol(CHOL),triglyceride(TG),Anti-PLA-2R,urinary podocyte apical membrane protein creatinine ratio(PCX/Cr),and 24 h urinary protein quantification(UTP)of the observation group are lower than those of the pre-treatment and control groups,and blood serum albumin(ALB)is higher than that of the pre-treatment and control groups,with statistically significant differences(P<0.05,P<0.01);the SOD of the observation group is higher than that of pre-treatment and control groups,and MDA,SOCS1,SOCS3 are all lower than those of pre-treatment and control groups,with statistically significant differences(P<0.05,P<0.01).Conclusion Yiqi Huoxue Bushen Prescription can improve the oxidative stress state,increase the urinary protein remission rate,and improve the clinical efficacy of patients with membranous nephropathy.
