摘要
目的 探究腹腔镜下胆管结石胆道镜切开取石一期缝合并内支架引流的手术方式对患者治疗和预后的影响。方法 连续性纳入我院自2020年1月至2021年12月我院收治的所有胆总管结石患者,共124例。所有患者均使用腹腔镜下胆道镜胆总管切开取石术治疗,根据术中对切开胆总管后的处置方式分为两组,一期缝合合并内支架引流为研究组,共37例患者,T管引流者为对照组,患者共87例。收集两组患者术前一般情况、围手术期指标、并发症的发生及出院后结石的复发率。结果 研究组患者术中出血量,术后结石复发率显著低于对照组,差异具有统计学意义(P<0.05)。研究组患者的手术时间、术后腹腔引流管留置时间、出院时间及并发症的发病率上均不劣于对照组。结论 应用腹腔镜下胆道镜切开取石术治疗胆管结石是行之有效的治疗方案,采取一期缝合合并内支架引流的方案是安全有效的,或能给患者带来更大获益。
Objective To investigate the impact of laparoscopic cholangiolithotomy,primary suture,and internal stent drain-age on the treatment and prognosis of patients with bile duct stones.Methods A total of 124 patients with common bile duct stones admitted to our hospital from January 2020 to December 2021 were included consecutively.All patients were treated with laparoscopic choledochoscopy choledocholithotomy and lithotomy.They were divided into two groups based on the treatment of the incision of the common bile duct during the surgery.The first stage suture combined with internal stent drainage was used as the study group,with a total of 37 patients,and the T-tube drainage was used as the control group,with a total of 87 patients.Collect general preoperative in-formation,perioperative indicators,incidence of complications,and recurrence rate of stones after discharge from two groups of pa-tients.Results The intraoperative bleeding volume and postoperative stone recurrence rate in the study group were significantly lower than those in the control group,with statistical significance(P<0.05).The operation time,retention time of postoperative peritoneal drainage tube,discharge time and incidence rate of complications in the study group were not inferior to those in the control group.Conclusion The application of laparoscopic cholangioscopy for the treatment of bile duct stones is an effective treatment plan.Adop-ting a one-stage suture combined with internal stent drainage is safe and effective,and may bring greater benefits to patients.
