摘要
目的探讨腹腔镜下胆总管探查术(LCBDE)后一期缝合在胆管结石患者中的临床应用效果.方法选取2019年8月—2021年10月该院收治的接受LCBDE治疗的86例胆管结石患者为研究对象,按照术后处理方式的不同分为两组.对照组(n=43)采用T管引流,观察组(n=43)采用一期缝合.比较两组手术时间、术后输液量、术后排气时间、术后住院时间、炎性因子指标水平以及术后并发症发生情况.结果观察组手术时间、术后排气时间、术后住院时间均短于对照组,术后输液量少于对照组,组间差异有统计学意义(P<0.05).术后5 d,观察组血清肿瘤坏死因子-α、白细胞计数以及C反应蛋白水平均低于对照组,组间差异有统计学意义(P<0.05).观察组术后并发症发生率4.65%低于对照组的18.60%,差异有统计学意义(P<0.05).结论临床对接受LCBDE治疗的胆管结石患者行术后一期缝合处理,能够有效减轻机体微炎症状态,减少并发症发生,促进患者胃肠功能恢复.
Objective To investigate the clinical application effect of one-stage suture after laparoscopic common bile duct exploration(LCBDE)in patients with bile duct stones.Methods 86 patients with bile duct stones who received LCBDE treatment in the hospital from August 2019 to October 2021 were selected as the research objects,and divided into two groups according to different postoperative treatment methods.The control group(n=43)used T tube drainage,and the observation group(n=43)used one-stage suture.The operation time,postoperative infusion volume,postoperative exhaust time,postoperative hospital stay,inflammatory factor index levels and postoperative complications were compared between the two groups.Results The operation time,postoperative exhaust time,and postoperative hospital stay in the observation group were shorter than those in the control group,and the postoperative infusion volume was less than that in the control group,and the differences between the groups were statistically significant(P<0.05).5 days after operation,the serum tumor necrosis factor-α,white blood cell count and C-reactive protein levels in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 4.65%,which was lower than 18.60%in the control group,and the difference was statistically significant(P<0.05).Conclusion Clinically,one-stage suture treatment for patients with bile duct stones treated with LCBDE can effectively reduce the micro-inflammatory state of the body,reduce the occurrence of complications,and promote the recovery of patients'gastrointestinal function.
