瑞马唑仑对老年髋关节置换术患者苏醒质量和炎症应激及术后谵妄发生率的影响研究

摘要探讨瑞马唑仑对老年髋关节置换术患者苏醒质量、炎症应激反应和术后谵妄发生率的影响。选取2023年1月—2024年1月西安交通大学第一附属医院榆林医院收治的80例接受老年髋关节置换术治疗的患者为研究对象,将其随机分为试验组和对照组,每组各40例。两组患者均采用相同的麻醉和手术方案,试验组在麻醉诱导前10 min静脉注射瑞马唑仑0.1 mg/kg,对照组静脉注射等容量的生理盐水。比较两组患者的一般资料、苏醒质量、血清IL-6、TNF-α水平以及术后谵妄发生率。结果显示,试验组患者的苏醒时间、气管拔管时间、定向力恢复时间、MMSE评分均优于对照组(P<0.05)。术后6 h、24 h,试验组患者的血清IL-6、TNF-α水平均低于对照组(P<0.05)。研究发现,瑞马唑仑可以有效提升老年髋关节置换术后患者的苏醒质量,降低患者的炎症应激反应水平以及术后谵妄的发生率。 Explore the effects of remifentanil on the quality of recovery,inflammatory stress response,and incidence of postoperative delirium in elderly patients undergoing hip replacement surgery.Selecting 80 elderly patients who underwent hip replacement surgery at Yulin Hospital of the First Affiliated Hospital of Xi’an Jiaotong University from January 2023 to January 2024 as the research subjects,they were randomly divided into an experimental group and a control group,with 40 patients in each group.Both groups of patients were given the same anesthesia and surgical protocol.The experimental group received intravenous injection of remifentanil 0.1 mg/kg 10 minutes before anesthesia induction,while the control group received intravenous injection of an equal volume of physiological saline.Compare the general information,recovery quality,serum IL-6,TNF-αlevels,and postoperative delirium incidence between two groups of patients.The results showed that the awakening time,tracheal extubation time,directional force recovery time,and MMSE score of the experimental group patients were all better than those of the control group(P<0.05).At 6 and 24 hours after surgery,the average levels of serum IL-6 and TNF-αin the experimental group were lower than those in the control group(P<0.05).Research has found that remifentanil can effectively improve the recovery quality of elderly patients after hip replacement surgery,reduce their inflammatory stress response,and lower the incidence of postoperative delirium.
