摘要
逆行性种植体周围炎(Retrograde peri-implantitis,RPI)是指在种植体植入术后短期内出现疼痛、肿胀、叩痛、瘘管等临床症状的一类根尖周病变。铒激光(Erbium yttrium-aluminium-garnet laser,Er∶YAG)能在种植体周围炎治疗中发挥有效的机械清洁作用,明显改善种植体周软组织的龈沟出血指数、探诊深度等,同时促进软硬组织的愈合和再生。目前,关于Er∶YAG在RPI的临床研究较少,本文结合近年来国内外研究文献,对Er∶YAG在RPI的预防及治疗中的应用进行综述。文献复习结果表明,Er∶YAG在RPI早期预防中的应用主要是种植位点保存、即刻种植以及邻牙显微根尖手术。Er∶YAG在RPI的治疗中也有广阔的应用,主要用于根尖病变处理、牙槽骨修整术、引导性骨再生以及术区残余细菌的清理等方面。
Retrograde peri-implantitis(RPI)refers to a type of periapical disease with clinical symptoms such as pain,swelling,percussion and fistula in the short term after implant implantation.Erbium yttrium-aluminium-garnet laser(Er∶YAG)can play an effective mechanical cleaning role in the treatment of peri-implantitis.It can significantly improve the sulcus bleeding index and probing depth of peri-implant soft tissues,and promote the healing and regeneration of soft and hard tissues.At present,there are few clinical studies on Er∶YAG in RPI.This review critically examines the utilization of Er∶YAG in the prevention and treatment of RPI,synthesizing recent research literature from both domestic and international sources,with the objective of introducing innovative perspectives for clinical diagnosis and management of RPI.The literature review showed that the application of Er∶YAG in the early prevention of retrograde peri-implantitis,mainly included implant site preservation,immediate implant and endodontic microsurgery.Er∶YAG has also been widely used in the treatment of RPI,mainly for apical lesion treatment,alveolar bone dressing,guided bone regeneration and residual bacteria cleaning in the surgical area.
