摘要
目的探讨脑卒中患者健康信念水平与主要照护者照护负担的相关性,为提升临床照护效果提供帮助。方法通过便利取样法选取2018年12月—2019年12月于天津市两家三级医院神经内科病区住院治疗的180例脑卒中患者及其主要照护者180例为研究对象。采用自行设计的一般资料问卷、脑卒中患者的Champion健康信念量表(CHBMS)以及Zarit照顾者负担量表(ZBI)进行问卷调查。结果本次调查共回收有效问卷174份,有效回收率为96.67%。脑卒中患者CHBMS总分为(103.86±8.60)分,各条目均分为(2.97±0.25)分。照护者ZBI总分为(24.37±18.04)分,其中个人负担维度得分为(15.46±9.72)分,高于角色负担的(3.57±5.60)分,差异有统计学意义(t=23.663,P<0.05)。相关分析显示,照护者ZBI与脑卒中患者CHBMS水平呈负相关(r=-0.655,P<0.01)。结论脑卒中患者健康信念整体处于中下水平,护理人员应在日常工作中采取有效措施为其提供有针对性的健康教育与指导,改善其健康信念水平,从而有效减轻主要照顾者的照护负担。
Objective To investigate the correlation between the level of health beliefs of stroke patients and the care burden of primary caregivers,in order to provide assistance for improving clinical care effect.Methods Through convenience sampling method,180 patients with stroke hospitalized in the department of neurology ward of two tertiary hospitals in Tianjin from December 2018 to December 2019 and their primary caregivers were selected as the research subjects.Self-designed general information questionnaire,Champion health belief scale(CHBMS)of stroke patients and Zarit caregiver burden interview(ZBI)were used to conduct the questionnaire survey.Results In the survey,174 effective questionnaires were returned,the effective recovery rate was 96.67%.The total score of CHBMS was(103.86±8.60)points,items average score was(2.97±0.25)points.The total score of ZBI of caregivers was(24.37±18.04)points,the score of personal burden dimension was(15.46±9.72)points,which was higher than(3.57±5.60)points of role burden dimension,and the difference was statistically significant(t=23.663,P<0.05).The relevant analysis showed that the ZBI of caregivers was negatively correlated with the level of CHBMS of stroke patients(r=-0.655,P<0.01).Conclusion The health belief of patients with stroke was at a lower-middle level.Nursing staff should take effective measures in their daily work to provide targeted health education and guidance,improve their health belief level,and effectively reduce the care burden of primary caregivers.
