摘要
目的分析颈部血管彩超联合血清微小RNA-222-3p(miR-222-3p)、微小RNA-206(miR-206)在脑梗死患者颈动脉狭窄程度诊断中的应用价值。方法选取该院2018年7月至2022年7月收治的90例脑梗死患者为研究组,并根据患者颈动脉狭窄程度分为轻度狭窄组(29例)、中度狭窄组(35例)、重度狭窄组(26例)。另选取在该院体检的96例体检健康者为对照组。采用实时荧光定量PCR对各组血清miR-222-3p、miR-206水平进行检测,脑梗死患者均接受颈部血管彩超检查,并分析不同狭窄程度患者临床资料。采用多因素Logistic回归分析影响脑梗死患者颈动脉狭窄程度的因素,受试者工作特征(ROC)曲线评估颈动脉超声指标及血清miR-222-3p、miR-206水平对脑梗死患者颈动脉狭窄程度的诊断价值。结果与对照组比较,研究组血清miR-222-3p水平升高,血清miR-206水平降低(P<0.05)。与轻度狭窄组比较,中度狭窄组、重度狭窄组收缩期峰值流速(PSV)、舒张末期容积(EDV)及血清miR-222-3p水平升高,血清miR-206水平降低(P<0.05);与中度狭窄组比较,重度狭窄组EVD、PSV及血清miR-222-3p水平升高,miR-206水平降低(P<0.05)。EVD、PSV、血清miR-222-3p水平是脑梗死患者发生颈动脉狭窄的危险因素,血清miR-206水平是脑梗死患者发生颈动脉狭窄的保护因素(P<0.05)。EVD、PSV联合血清miR-222-3p、miR-206诊断脑梗死患者颈动脉狭窄程度的曲线下面积为0.991,灵敏度和特异度分别为98.36%和93.10%,优于EVD、PSV、血清miR-222-3p、miR-206单独诊断(Z_(四者联合-EVD)=2.927、P=0.003,Z_(四者联合-PSV)=2.834、P=0.005,Z_(四者联合-miR-222-3p)=3.300、P=0.001,Z_(四者联合-miR-206)=3.730、P<0.001)。结论颈部血管彩超联合血清miR-222-3p、miR-206在脑梗死患者颈动脉狭窄程度诊断中具有应用价值。
Objective To analyze the application value of cervical vascular color Doppler ultrasound combined with serum microRNA-222-3p(miR-222-3p)and microRNA-206(miR-206)in the diagnosis of carotid artery stenosis in patients with cerebral infarction.Methods A total of 90 patients with cerebral infarction admitted to this hospital from July 2018 to July 2022 were selected as the study group,and were divided into mild stenosis group(29 cases),moderate stenosis group(35 cases),and severe stenosis group(26 cases)according to the degree of carotid artery stenosis.In addition,96 healthy people who underwent physical examination in the hospital were selected as the control group.Real-time fluorescence quantitative PCR was used to detect the levels of serum miR-222-3p and miR-206 in each group.All patients with cerebral infarction underwent cervical vascular color Doppler ultrasound examination,and the clinical data of patients with different degrees of stenosis were analyzed.Multivariate Logistic regression was used to analyze the factors affecting the degree of carotid artery stenosis in patients with cerebral infarction.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of carotid ultrasound indicators,serum miR-222-3p and miR-206 levels for the degree of carotid artery stenosis in patients with cerebral infarction.Results Compared with the control group,the study group had a significant increase in the serum level of miR-222-3p and a significant reduction in the serum level of miR-206(P<0.05).Compared with the mild stenosis group,the peak systolic velocity(PSV),end diastolic volume(EDV)and serum miR-222-3p level were increased,and the serum miR-206 level was decreased in the moderate stenosis group and the severe stenosis group(P<0.05).Compared with moderate stenosis group,EVD,PSV and serum miR-222-3p levels were significantly increased,and miR-206 level was decreased in severe stenosis group(P<0.05).EVD,PSV,serum miR-222-3p level were risk factors for carotid artery stenosis in patients with cerebral infarction,and serum miR-206 level was a protective factor for carotid artery stenosis in patients with cerebral infarction(P<0.05).The area under the curve of EVD,PSV combined with serum miR-222-3p and miR-206 in diagnosing the degree of carotid artery stenosis in patients with cerebral infarction was 0.991,and the sensitivity and specificity were 98.36%and 93.10%,respectively.It was better than that of EVD,PSV,serum miR-222-3p,and miR-206 alone(Z_(combination-EVD)=2.927,P=0.003,Z_(combination-PSV)=2.834,P=0.005,Z_(combination-miR-222-3p)=3.300,P=0.001,Z combination-miR-206=3.730,P<0.001).Conclusion Cervical vascular color Doppler ultrasound combined with serum miR-222-3p and miR-206 has application value in the diagnosis of carotid artery stenosis in patients with cerebral infarction.
