摘要
目的:观察在牙周炎致前牙扇形移位患者中应用无托槽隐形矫治器的效果。方法:选择2020年9月—2023年6月于上海市虹口区江湾医院接受治疗的牙周炎致前牙扇形移位患者88例,随机将患者分为两组,对照组(44例)予以自锁托槽矫治器治疗,观察组(44例)予以无托槽隐形矫治器治疗。观察两组临床疗效、牙周指标、炎症因子水平及美学效果。结果:观察组总有效率为97.44%,明显较对照组的76.92%高,差异有统计学意义(P<0.05)。治疗6个月后,观察组牙周探诊深度(PD)、附着丧失(AL)、龈沟出血指数(SBI)、牙龈指数(GI)、菌斑指数(PLI)、白细胞介素-6(IL-6)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)水平明显均较对照组低,差异均有统计学意义(P<0.05)。治疗6个月后,观察组咬合力高于对照组,牙齿美观指数评分低于对照组,差异均有统计学意义(P<0.05)。结论:在牙周炎致前牙扇形移位患者中,应用无托槽隐形矫治器的疗效显著,可有效改善牙周情况,减轻炎症反应,且美学效果良好。
Objective:To observe the effect of applying invisible orthosis without bracket in patients with anterior teeth fan-shaped displacement caused by periodontitis.Method:A total of 88 patients with anterior teeth fan-shaped displacement in Jiangwan Hospital,Hongkou District,Shanghai City were selected from September 2020 to June 2023.They were randomly divided into two groups,the control group(44 cases)was treated with self-locking bracket orthosis,the observation group(44 cases)was treated with invisible orthosis without bracket.The clinical efficacy,periodontal index,inflammatory factor level and aesthetic effect of the two groups were observed.Result:The total effective rate in the observation group was 97.44%,which was higher than 76.92%in the control group,the difference was statistically significant(P<0.05).After 6 months of treatment,gingival index(GI),sulcus bleeding index(SBI),plaque index(PLI),attachment loss(AL),periodontal depth(PD),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-4(IL-4)in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After 6 months of treatment,bite force in the observation group was higher than that in the control group,and dental aesthetic index score was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:In patients with anterior teeth fan-shaped displacement caused by periodontitis,the application of without bracket invisible orthosis is effective,which can effectively improve the periodontal condition,reduce inflammation response,and have good aesthetic effect.
