摘要
目的:探讨自拟消渴膏联合胰岛素治疗糖尿病酮症酸中毒对酮体转阴时间及血糖水平的影响。方法:选取2020年3月~2023年12月在我院就诊的60例糖尿病酮症酸中毒患者进行研究。将上述患者录入计算机系统,随机排序,使用抽签法将前30序号患者分为对照组给予西医常规胰岛素治疗,后30序号患者分为观察组额外给予自拟消渴膏治疗。观察两组患者的临床疗效、病情控制情况、中医症候积分、血糖水平、炎症因子以及并发症情况。结果:观察组患者的治疗有效率93.33%高于对照组患者73.33%(P<0.05);观察组患者各病情控制情况指标均短于对照组患者(P<0.05);治疗后,观察组患者的精神萎靡、四肢无力、胸闷气短、口干口渴积分、24h平均血糖水平(24hMBG)、平均血糖波动幅度(MAGE)、最大血糖波动幅度(LAGE)、白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(CRP)均低于对照组患者(P<0.05);观察组患者并发症率3.33%低于对照组患者20.00%(P<0.05)。结论:自拟消渴膏联合胰岛素治疗糖尿病酮症酸中毒患者疗效良好,可以缩短酮体转阴时间,改善患者主要症状、血糖水平、炎症因子并降低并发症率。
Objective:To explore the effects of self-made Xiaoke ointment combined with insulin on negative conversion time of ketone body and blood glucose level in diabetic ketoacidosis.Methods:A total of 60 patients with diabetic ketoacidosis treated in the hospital were enrolled between March 2020 and December 2023.The patients were put into the computer system and randomly ranked.According to lottery method,patients were divided into control group(routine western medicine insulin)and observation group(self-made Xiaoke ointment+insulin),30 cases in each group.The clinical curative effect,disease control,scores of TCM syndromes,blood glucose level,inflammatory factors and complications in the two groups were observed.Results:The response rate of treatment in observation group was higher than that in control group(93.33%vs 73.33%,P<0.05).The time of disease control indexes were shorter in observation group than control group(P<0.05).After treatment,scores of TCM syndromes(listlessness,limbs weakness,chest tightness and shortness of breath,dry mouth and thirst),24h mean blood glucose(24hMBG),mean amplitude of glycemic excursion(MAGE),largest amplitude of glycemic excursion(LAGE),levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and hypersensitive C-reactive protein(CRP)were lower in observation group than control group(P<0.05).The incidence of complications in observation group was lower than that in control group(3.33%vs 20.00%,P<0.05).Conclusion:The curative effect of self-made Xiaoke ointment combined with insulin is good in patients with diabetic ketoacidosis,which can shorten negative conversion time of ketone body,improve main symptoms,blood glucose,inflammatory factors and reduce the incidence of complications.
