摘要
目的探讨乌司他丁联合皮下胰岛素泵注治疗糖尿病酮症酸中毒的效果。方法2022年1月—2023年12月遂宁市中心医院急诊收治的糖尿病酮症酸中毒患者,按照不同的治疗方法分为两组。对照组75例采用皮下胰岛素泵注及基础治疗,研究组77例在上述基础上加用乌司他丁治疗。治疗7 d后评价疗效,比较血pH、血酮体、空腹血糖、餐后2 h血糖、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)、不良反应。结果研究组75例,对照组77例,研究组总有效率高于对照组(92.00%vs.80.52%,P<0.05)。研究组血pH高于对照组,研究组血清酮体水平低于对照组(P<0.05)。研究组的空腹血糖、餐后2 h血糖低于对照组(P<0.05)。研究组的血浆SOD、GSH-PX水平高于对照组(P<0.05)。研究组与对照组的不良反应率差异无统计学意义(P>0.05)。结论乌司他丁联合皮下胰岛素泵注治疗酮症酸中毒可明显纠正酸中毒,降低血酮和血糖,改善机体氧化应激状态,增加临床获益。
Objective To explore the effect of ulinastatin combined with subcutaneous insulin pumping in the treatment of diabetic ketoacidosis.Methods A total of 152 patients with diabetic ketoacidosis who were admitted to the emergency department in Suining Central Hospital from January 2022 to December 2023 were divided into groups by prospective randomization.Patients in the control group were treated with subcutaneous insulin pumping and basic treatment,and patients in the study group were treated with ulinastatin combined with insulin pumping and basic treatment.After 7 days of treatment,the efficacy was evaluated,and blood pH,blood ketone body,fasting blood glucose,2 h postprandial blood glucose,superoxide dismutase(SOD),glutathione peroxidase(GSH-PX),and adverse reactions were compared.Results Study group(n=75)and control group(n=77).Compared with the control group,the total effective rate of the study group was higher(92.00%vs.80.52%,P<0.05).Compared with the control group,the blood pH,plasma SOD and GSH-PX levels in the study group was higher,and ketone levels,fasting blood glucose and 2 h postprandial blood glucose was lower(P<0.05).The adverse reaction rates of the study group and the control group was not statistically significant(P>0.05).Conclusion The treatment of ketoacidosis with ulinastatin combined with insulin pumping can significantly correct acidosis,reduce blood ketones and blood glucose,improve oxidative stress and increase clinical benefits.
