血清CRP、Hcy水平与老年2型糖尿病患者维生素D缺乏的相关性分析 被引量:1

摘要 目的探讨分析老年2型糖尿病患者(T2DM)血清C-反应蛋白(CRP)、同型半胱氨酸(Hcy)水平与维生素D缺乏(VDD)的相关性。方法选取2018年9月至2022年8月在甘肃省人民医院内分泌科住院的439例T2DM患者为研究对象,分为老年男性组(238例)和老年女性组(201例)。对比两组VDD患病率,通过相关分析及二元Logistic回归分析,评估炎症因子CRP、Hcy水平与VDD的相关性,总结VDD的危险因素。结果两组空腹血糖(FBG)、糖化血红蛋白(HbA1c)、C反应蛋白(CRP)、尿酸(UA)等对比,P>0.05;两组VDD患病率对比,P>0.05;两组患者血清25-羟维生素D[25-(OH)D]水平与糖尿病病程呈正相关,与CRP、Hcy、PTH水平均呈负相关。CRP和Hcy是VDD的危险因素,糖尿病病程是保护性预测因子(P<0.05)。结论血清CRP、Hcy水平与25-(OH)D负相关,监测二者的水平对识别及干预VDD有一定的临床意义。 Objective:To explore the correlation between serum levels of C-reactive protein(CRP)and homocysteine(Hcy)and vitamin D deficiency(VDD)in elderly patients with type 2 diabetes mellitus(T2DM).Methods:From September 2018 to August 2022,439 patients with T2DM hospitalized in the Department of Endocrinology of Gansu Provincial People's Hospital were selected as the research objects,and they were divided into two groups:the elderly male group(238 cases)and the elderly female group(201 cases).The prevalence rate of VDD was compared between the two groups.Through correlation analysis and binary Logistic regression analysis,the correlation between the levels of inflammatory factors CRP and Hcy and VDD was evaluated,and the risk factors of VDD were summarized.Results:Fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c)、C-reactive protein(CRP)and uric acid(UA)were compared between the two groups,P>0.05.The prevalence of VDD in the two groups was compared,P>0.05;The level of serum 25-hydroxyvitamin D[25-(OH)D]was positively correlated with the course of diabetes,and negatively correlated with the levels of CRP、Hcy and PTH.CRP and Hcy are risk factors for VDD,and the course of diabetes mellitus is a protective predictor,P<0.05.Conclusion:Serum CRP and Hcy levels are negatively correlated with 25-(OH)D.Monitoring the levels of CRP and Hcy has certain clinical significance for identifying and intervening VDD.
