摘要
目的:通过数据挖掘技术分析皇运祥主任中医师治疗虚劳的病案,探讨其治疗虚劳的用药特点及组方规律。方法:采用Microsoft Excel对2022年1月至2023年6月皇运祥主任中医师治疗虚劳所用中药及其属性进行频次分析,采用SPSS Modeler 18.0软件的Apriori算法对处方组成药物进行关联规则分析,使用SPSS Statistic 25.0进行系统聚类分析,寻求高频药物组合。结果:共纳入虚劳患者病案315例,患者舌象主要为舌淡白、苔腻,脉象以滑脉为主;证型以脾虚痰湿证最为常见;共纳入中药处方515首,中药204味,药物多性温、味甘、归脾经;使用频率最高的药物为茯苓,药物类别主要是补虚药中的补气药。关联规则中支持度最高的为茯苓-白术,置信度最高的为茯苓-砂仁-陈皮。对高频药物进行聚类分析,共提取到3个新处方。结论:皇运祥主任中医师治疗虚劳的处方具有鲜明的用药规律,辨证分型以脾虚痰湿证为主,在五脏虚损上重视脾肾,用药侧重甘温扶阳,治疗多以补虚、渗湿立法,辅以收涩、理气之药。
Objective:To investigate the medication and prescription rules of Chief Traditional Chinese Medicine(TCM)Physician Huang Yunxiang in the treatment of consumptive disease by analyzing related medical records based on data mining.Methods:Microsoft Excel was used to perform a frequency analysis of TCM drugs and their attributes used by Chief TCM Physician Huang Yunxiang in the treatment of consumptive disease from January 2022 to June 2023,the Apriori algorithm in SPSS Modeler18.0 was used to perform an association rule analysis of the drugs in prescriptions,and SPSS Statistic 25.0 was used for hierarchical cluster analysis to search for high-frequency drug combinations.Results:The medical records of 315 patients with consumptive disease were included.Pale tongue and greasy coating were the main tongue manifestations,and slippery pulse was the main pulse manifestation.Spleen deficiency and phlegm-dampness syndrome was the most common syndrome type.A total of 515 prescriptions were included,involving 204 TCM drugs,among which most drugs had a warm nature and a sweet taste and entered the spleen meridian.Poria cocos had the highest frequency of use,and Qi-tonifying drugs among deficiency-tonifying herbs were the main type of drug.The association rule analysis showed the highest degree of support in Poria cocos-Atractylodes macrocephala Koidz.And the highest degree of confidence in Poria cocos-Amomum villosum-tangerine peel.A cluster analysis was performed for high-frequency drugs,and three new prescriptions were extracted.Conclusion:Chief TCM Physician Huang Yunxiang has unique medication rule in the treatment of consumptive disease,with the main syndrome type of spleen deficiency and phlegm-dampness syndrome,and the spleen and the kidneys are taken seriously in terms of the deficiency and damage of five viscera.Sweet and warmnatured drugs for supporting Yang are mainly used for treatment,and tonifying deficiency and draining dampness are the main treatment methods,with the assistance of drugs for inducing astringency and regulating Qi.
