摘要
目的观察二仙汤加减联合舍曲林治疗老年肿瘤相关抑郁症(ETRD)患者临床疗效。方法将我院60例ETRD患者随机分为治疗组与对照组各30例。对照组应用舍曲林治疗,治疗组应用二仙汤加减联合舍曲林治疗。比较两组采用汉密尔顿抑郁量表(HAMD)评分评价的临床疗效,比较两组抑郁症筛查量表(PHQ-9)、HAMD、匹兹堡睡眠质量指数量表(PSQI)评分、生活质量问卷(QLQC30)中4个维度、5-羟色胺(5-HT)、去甲肾上腺素(NE)、白细胞介素-6(IL-6)水平、CD3^(+)、CD4^(+)、CD8^(+)T淋巴细胞亚群百分数。结果治疗后,治疗组总有效率显著高于对照组(P<0.05);两组PHQ-9、HAMD、PSQI评分低于治疗前,治疗组PHQ-9、HAMD、PSQI评分低于对照组(P<0.05);两组QLQC30中4个维度评分高于治疗前,治疗组QLQC30中4个维度评分高于对照组(P<0.05);两组5-HT、NE水平高于治疗前,IL-6水平低于治疗前;治疗组5-HT、NE水平高于对照组,IL-6水平低于对照组(P<0.05);两组CD3^(+)、CD4^(+)百分数高于治疗前,CD8^(+)百分数低于治疗前;治疗组CD3^(+)、CD4^(+)百分数高于对照组,CD8^(+)百分数低于对照组(P<0.05)。结论二仙汤加减联合舍曲林可显著减轻ETRD患者的抑郁症状,改善睡眠和生活质量,提高单胺类神经递质水平,下调炎性因子水平,提高患者免疫力。
Objective To observe the therapeutic effect of modified Erxian Decoction combined with sertraline in treatment of elderly patients with tumor related depression(ETRD).Methods 60 patients with ETRD were randomly divided into treatment group and control group,with 30 cases in each group.Control group received sertraline,while treatment group received modified Erxian Decoction on the basis of control group.The efficacy of two groups of Hamilton Depression Rating Scale(HAMD),Depression Screening Scale(PHQ-9),HAMD,Pittsburgh Sleep Quality Index(PSQI)scores,four dimensions of Quality of Life Questionnaire(QLQC30),serotonin(5-HT),norepinephrine(NE),interleukin-6(IL-6)levels,CD3^(+),CD4^(+),and CD8^(+)T lymphocyte subsets percentage were compared.Results After treatment,the total effective rate of HMAD in treatment group was significantly higher than that in control group(P<0.05);the PHQ-9,HAMD,and PSQI scores of both groups were lower than those before treatment,and were lower in treatment group than control group(P<0.05);four dimensions scores in both groups of QLQC30 were higher than before treatment,and were higher in treatment group than control group(P<0.05);5-HT and NE levels in both groups were higher than before treatment,and were higher in treatment group than control group,while IL-6 levels were lower in both groups than before treatment,and were lower in observation group than control group(P<0.05);the percentage of CD3^(+)and CD4^(+)in both groups were higher than before treatment,and was higher in treatment group than control group,while CD8^(+)percentage was lower in both groups than before treatment,and was lower in observation group than control group(P<0.05).Conclusion Modified Erxian Decoction combined with sertraline can significantly reduce depressive symptoms in ETRD patients,improve sleep and quality of life,increase the levels of monoamine neurotransmitters factors,decrease inflammatory factors,and enhance patients’immunity.
