摘要
目的 探究滋肾补脾益气合祛湿活血化浊汤治疗脾肾气虚兼湿浊血瘀证糖尿病肾病(Diabetes nephropathy,DN)患者的临床价值。方法 选取2021年6月—2022年12月期间北京中医医院怀柔医院收治的DN患者90例,依据简单随机数字表法分为对照组和研究组,每组各45例。常规干预基础上对照组采取西医治疗,研究组在对照组基础上采取滋肾补脾益气合祛湿活血化浊汤,均治疗1个月。观察比较两组患者临床疗效、不良反应发生情况,治疗前后中医证候积分、肾功能[血肌酐(Serum creatinine,Scr)、血清尿素氮(blood urea nitrogen,BUN)、24 h尿蛋白定量]水平、血清同型半胱氨酸(Homocysteine,Hcy)及白细胞介素-6(Interleukin-6,IL-6)水平。结果 治疗后研究组临床总有效率93.33%(42/45)明显高于对照组77.78%(35/45),差异有统计学意义(P<0.05)。治疗后两组患者主症及次症分值均较治疗前降低,差异有统计学意义(P<0.05);且研究组主症及次症分值均明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者Scr、BUN、24 h尿蛋白定量均较治疗前降低,差异有统计学意义(P<0.05);且研究组Scr、BUN、24 h尿蛋白定量均明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清Hcy及IL-6水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组血清Hcy及IL-6水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗期间,研究组不良反应发生率8.89%(4/45)与对照组4.44%(2/45)比较,差异无统计学意义(P>0.05)。结论 常规西药联合滋肾补脾益气合祛湿活血化浊汤治疗DN,可有效缓解患者临床症状,改善肾功能,下调Hcy及IL-6水平,利于提升整体治疗效果,且具有安全性。
Objective To evaluate the clinical significance of Zishen Bupi Yiqi and Qushi Huoxue Huazhuo Decoction in treating diabetic nephropathy(DN)patients with spleen-kidney qi deficiency and dampness-turbidity blood stasis.Methods Ninety DN patients at Beijing Huairou Hospital of of Traditional Chinese Medicine from June 2021 to December 2022 were selected and assigned to a study group and a control group by random number table,with 45 in each group.The control group received standard Western treatment,while the study group received additional Zishen Bupi Yiqi and Qushi Huoxue Huazhuo Decoction on the basis of the control group.Both groups underwent one month of treatment.The following outcomes were assessed:clinical efficacy,adverse reactions,TCM syndrome scores,kidney function indexes[serum creatinine(Scr),blood urea nitrogen(BUN),24-h urinary protein quantification],serum homocysteine(Hcy),and interleukin-6(IL-6).Results The total effective rate was 93.33%(42/45)in the study group,which was higher than the 77.78%(35/45)in the control group(P<0.05).After treatment,both groups demonstrated a decrease in the scores for primary and secondary symptoms(P<0.05),with the study group displaying lower scores than the control group(P<0.05).Kidney function was improved in both groups following treatment(P<0.05),and the improvements were more pronounced in the study group(P<0.05).Serum Hcy and IL-6 also decreased after treatment in both groups(P<0.05),with greater reductions in the study group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study and control groups during treatment(8.89%,4/45 vs 4.44%,2/45,P>0.05).Conclusion Conventional Western medicine combined with Zishen Bupi Yiqi and Qushi Huoxue Huazhuo Decoction is effective in alleviating clinical symptoms,improving kidney function,reducing Hcy and IL-6 of DN,and is thus beneficial for enhancing overall treatment results while maintaining safety.
