摘要
酒渣鼻之病名和理论肇始于先秦时期,《黄帝内经》对其病因病机有详细描述,目前认为本病是由于肺胃积热,毒瘀之邪结聚,上泛于鼻面部而发病。毒邪理论滥觞于仲景,毒邪是邪气蕴结不解,“毒”在酒渣鼻的发生发展过程中有重要影响,毒瘀内蕴是酒渣鼻发病的关键。常见病因病机为寒毒致瘀、热毒致瘀、湿毒致瘀、痰瘀毒互结、久瘀痼疾等。寒毒致瘀多用通窍活血汤加减,同时多予辛温散寒之品;热毒致瘀多用大黄䗪虫丸加减;湿毒致瘀予祛湿邪之品配合少量活血化瘀药,方选除湿胃苓汤加减;痰瘀毒互结予活血化瘀软坚散结药物,如海藻、昆布、三棱、莪术等;久瘀痼疾常用虫类药配合固护脾胃、滋阴养血之品,常配伍白芍、枸杞子、丹参、生地黄等。
The name and theory of rosacea originated from the pre Qin period.Huang Di Nei Jing provides a detailed description of its etiology and pathogenesis.Currently,it is believed that this disease is caused by the accumulation of heat in the lung and stomach,the accumulation of toxins and blood stasis,and the spread of the disease on the nose and face.The theory of toxin and pathogens originated in Zhongjing.Toxin and pathogens are the inexplicable accumulation of evil energy,and“poison”has an important impact on the occurrence and development of rosacea.The accumulation of toxin and blood stasis is the key to the onset of rosacea.Blood stasis caused by cold toxin,blood stasis caused by heat toxin,blood stasis caused by dampness toxin,phlegm and blood stasis,chronic stasis and so on are the common etiology and pathogenesis.Blood stasis caused by cold toxin is mainly treated by modified Tongqiao Huoxue Decoction,with herbs acrid in flavour for dissipating cold;blood stasis caused by heat toxin is mainly treated by modified Dahuang Zhechong Pills;blood stasis caused by dampness toxin is mainly treated by herbs of reducing dampness with small amount of medicines of activating blood circulation and reducing blood stasis,with modified Chushi Weiling Decoction;phlegm and blood stasis is mainly treated by herbs of softening and disintegrating masses,such as Sargassum,Laminariae Thallus,Sparganii Rhizoma,Curcumae Rhizoma;chronic stasis is mainly treated by insect drugs,with herbs of strengthening spleen and stomach,nourishing yin and blood,and the common compatibility is Paeoniae Radix alba,Lycii Fructus,Salviea Miltiorrhizae Radix et Rhizoma,Rehmannine Radix,etc.
