摘要
目的:分析乌司他丁联合奥曲肽治疗急性胰腺炎的效果。方法:选取2022年8月—2023年11月四川省第三人民医院消化内科收治的急性胰腺炎患者90例作为研究对象,采用随机数字表法分为对照组与观察组,各45例。两组均行常规治疗,对照组在常规治疗基础上给予奥曲肽,观察组在对照组基础上给予乌司他丁。比较两组治疗效果、症状改善时间、住院时间、血清酶指标水平、炎性因子水平。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.026)。观察组腹痛腹胀缓解时间、肠鸣音恢复时间、首次排便时间早于对照组,住院时间短于对照组,差异有统计学意义(P<0.001)。治疗后,两组淀粉酶、脂肪酶水平降低,观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组白细胞介素-6、C反应蛋白、肿瘤坏死因子-α水平降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:乌司他丁联合奥曲肽治疗急性胰腺炎的效果确切,可缩短相关症状改善时间及住院时间,降低血清酶指标,减轻炎性反应。
Objective:To analyze the effect of ulinastatin combined with octreotide in the treatment of acute pancreatitis.Methods:A total of 90 patients with acute pancreatitis were selected as the research objects at the Gastroenterology Department of the Third People's Hospital of Sichuan Province from August 2022 to November 2023,and were divided into control group and observation group by random number table method,with 45 cases in each group.Both groups received conventional treatment,the control group was given octreotide treatment on the basis of conventional treatment,the observation group was given ulinastatin treatment on the basis of control group.The therapeutic effect,symptom improvement time,hospital stay,serum enzyme index level and inflammatory factor level were compared between the two groups.Results:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.026).The relief time of abdominal pain and distension,the recovery time of bowel sound and the time of first defecation in the observation group were earlier than those in the control group,and the length of hospitalization was shorter than that in the control group,the difference was statistically significant(P<0.001).After treatment,the levels of amylase and lipase in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-6,C-reactive protein and tumor necrosis factor-αin two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Ulinastatin combined with octreotide is effective in the treatment of acute pancreatitis.It can shorten the time of symptom improvement and hospital stay,reduce serum enzyme index and alleviate inflammatory response.
