摘要
目的探究腹部穴位按摩联合耳穴压豆干预在癌痛患者口服阿片类药物致药源性便秘中的应用效果。方法选取2021年1月至2022年12月广东省第二中医院收治的80例口服阿片类药物致药源性便秘患者作为研究对象,采用随机数字表法分为对照组与观察组,每组40例。观察组给予常规护理干预,观察组在对照组基础上行腹部穴位按摩联合耳穴压豆干预。比较两组临床疗效和干预前后中医症候积分、中文版便秘患者症状自评量表(PAC-SYM)评分、便秘患者生命质量自评问卷(PAC-QOL)评分及康复指标(首次排便时间、排便所需时间、排便间隔时间)。结果观察组临床总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(P<0.05)。干预后,两组排便无力、大便干结、排便周期延长、腹胀腹痛、四肢乏力、食少纳差积分均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。干预后,两组大便症状、腹部症状、直肠症状评分均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。干预后,两组生理、忧虑、社会心理、满意度评分均低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组首次排便时间、排便所需时间及排便间隔时间均短于对照组,差异有统计学意义(P<0.05)。结论腹部穴位按摩联合耳穴压豆干预癌痛患者口服阿片类药物致药源性便秘患者,可明显提升临床疗效,改善患者临床症状及严重程度,提升其生命质量,值得临床推广应用。
Objective To study the effect of abdominal acupoint massage combined with auricular pressing bean intervention in drug-induced constipation caused by oral opioids in patients with cancer pain.Methods 80 patients with drug-induced constipation caused by oral opioids in Guangdong Second Hospital of Traditional Chinese Medicine from January 2021 to December 2022 as the research subjects,and they were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The observation group was given routine nursing intervention,and the observation group was given abdominal acupoint massage combined with auricular pressing bean in-tervention based on the control group.The clinical efficacy,Traditional Chinese Medicine(TCM)symptom score,Chinese version of patient assess-ment of constipation symptom(PAC-SYM)and patient assessment of constipation quality of life questionnaire(PAC-QOL)before and after interven-tion and rehabilitation indexes(the first defecation time,defecation takes time and defecation interval time)were compared between the two groups.Results The total effective rate in the observation group was 95.00%,which was higher than 77.50%in the control group,and the difference was statistically significant(P<0.05).After the intervention,the scores of defecation weakness,dry stool,prolonged defecation cycle,abdominal disten-sion and abdominal pain,limb weakness,and food deficit difference of the two groups were lower than those before intervention,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).After intervention,the scores of stool symptoms,ab-dominal symptoms and rectal symptoms of the two groups were lower than before intervention,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).After intervention,the scores of physiology,worry,social psychology and satisfaction in both groups were lower than before intervention,and the observation group was lower than the control group,the differences were statistically signifi-cant(P<0.05).The first defecation time,defecation takes time and defecation interval time in the observation group were shorter than those in the con-trol group,the differences were statistically significant(P<0.05).Conclusion Abdominal acupoint massage combined with auricular pressing bean intervention in patients with cancer pain and drug-induced constipation caused by oral opioids can significantly improve the clinical efficacy,improve clinical symptoms and severity of patients,and improve the quality of life in patients,which is worthy of clinical promotion and application.
