L型钙离子通道α1D亚基基因多态性对苯磺酸氨氯地平治疗老年高血压患者效果的影响 被引量:3

A Clinical Study on the Effect of the Polymorphism of CACNA1Don Geriatric Hypertension Treatment with Amlodipine

摘要 目的探讨L型钙离子通道α1D亚基基因CACNA1D基因多态性对苯磺酸氨氯地平治疗老年原发高血压临床疗效的影响。方法选取2021年1月至2022年5月就诊于北京市西城区广外医院的101例老年原发高血压患者,7∶00 am口服苯磺酸氨氯地平5mg/d治疗8周,采集体质指数(BMI)、收缩压(SBP)、舒张压(DBP)信息。并检测尿素(Urea)、肌酐(Cr)、血糖(Glu)等指标。通过DNA提取和PCR扩增进行CACNA1D基因分型(rs312481、rs3774426、rs219847位点)。结果不同基因型患者(rs312481、rs3774426、rs219847位点)BMI及Urea等一般临床指标比较,差异无统计学意义(P>0.05)。rs312481的GG型舒张压降幅大于GA组,差异具有统计学意义(P<0.05),但与AA型比较无统计学差异(P>0.05)。rs219847中,GG组收缩压和舒张压降幅均大于GA组和AA组,且rs3774426中CT组收缩压和CC组舒张压降幅大于另外两组,但差异无统计学意义(P>0.05)。结论在氨氯地平治疗老年高血压患者中,CACNA1D基因rs312481位点GG型携带者相对于GA型可能会有更好的疗效。 Objective To evaluate the relationship between genetic polymorphism of CACNA1D that encoded the alpha1D subunit of the L-type calcium channel and amlodipine in senile essential hypertension(SEH).Methods For the 101 cases of SEH in Beijing Xicheng District Guangwai Hospital from January,2021 to May 2022,Amlodipine besylate 5mg/d was given orally at 7∶00 am for 8 weeks.Clinical records of body mass index(BMI),systolic blood pressure(SBP),and diastolic blood pressure(DBP)were collected.Other indicators,including urea nitrogen(Urea),creatinine(Cr),and blood glucose(Glu)were also tested.Genotyping of CACNA1D(rs312481,rs3774426,rs219847)was performed by DNA extraction and PCR amplification.Results There was no significant difference in general clinical indicators such as BMI and Urea in patients with different genotypes(rs312481,rs3774426,rs219847 locus)(P>0.05).DBP reduction in GG type of rs312481 was greater than that in GA group(P<0.05),but there was no statistically significant difference compared with AA type(P>0.05).In rs219847,both SBP and DBP reductions in the GG group were greater than those in GA and AA group,and SBP in the CT group and DBP in the CC group were greater than those in the other two groups in rs3774426,but with no statistically significant difference(P>0.05).Conclusion SEH patients with GG type of rs312481 in CACNA1D are more likely to be responders of amlodipine compare with GA type.
