摘要
功能性便秘(functional constipation,FC)是常见的一种胃肠功能紊乱疾病,该病发病机制复杂,脑-肠-菌轴(BGMA)在其病理生理中的作用逐渐受到关注,脑-肠与肠道菌群交互障碍可通过神经、内分泌、免疫和代谢等途径影响胃肠道运动、感觉、分泌等功能,进而引起排便异常。天枢穴具有疏调肠腑、理气行滞、健脾和胃、调神的功效,为针灸临床治疗FC的要穴。现代研究表明,针灸天枢穴对该病的影响机制可能与调节BGMA有关,包括促进中枢神经系统、肠神经系统结构与功能的正常化,改善肠道微生态失调,调节脑肠肽的分泌以及抑制促肾上腺皮质激素释放因子的过度表达等。该文从BGMA角度探究脑-肠轴、肠道菌群与FC的发病关系,总结探讨天枢穴治疗FC的内在机制,以期为针灸治疗功能性胃肠疾病提供理论依据和临床应用指导。
Functional constipation(FC)is a common functional gastrointestinal disorder and the pathogenesis of this disease is complex.Increasing attention has been paid to resech the role of brain-gutmicrobiota axis in its pathophysiology.The brain-gut and gut microbiota(BGMA)interaction disorders can affect gastrointestinal movement,sensation,secretion and other functions through neurological,endocrine,immune and metabolic pathways,and then cause abnormal defecation.Tianshu(ST25)is an important acupoint for the clinical treatment of FC in acupuncture,which has the effect of purging the large intestines,regulating Qi-flowing for activating stagnancy,strengthening the spleen and stomach and regulating the spirit.Recent studies indicate that the mechanism of acupuncture on the disease may be related to the regulation of BGMA,including promoting the recovery of the function and structure of the central and enteric nervous system,improving the dysbiosis of gut microbiota,modulating the secretion of brain-gut peptides and suppressing the overexpression of corticotropin-releasing factor.Based on the theory of BGMA,this paper explores the relationship between brain-gut axis,gut microbiota and FC,and summarizes the action mechanism of acupuncture for FC,in order to provide a theoretical basis for acupuncture in the treatment of functional gastrointestinal diseases and guide the clinical application.
