摘要
目的:对比腹腔镜下肠粘连松解术与开腹肠粘连松解术治疗小儿粘连性肠梗阻的效果。方法:选取2021年8月—2023年6月郓城县丁里长镇卫生院小儿粘连性肠梗阻患儿90例为研究对象,随机分为腹腔镜组和开腹组,各45例。腹腔镜组采用腹腔镜下肠粘连松解术,开腹组采用开腹肠粘连松解术。比较两组手术相关指标、临床疗效、术后并发症发生情况、复发情况。结果:腹腔镜组手术时间、术后清醒时间、肠鸣音恢复时间、胃肠功能恢复时间、肛门排气时间、腹痛消失时间、住院时间短于开腹组,术中出血量少于开腹组,差异有统计学意义(P<0.05);腹腔镜组临床疗效总有效率高于开腹组,差异有统计学意义(P=0.007);腹腔镜组术后并发症总发生率、复发率低于开腹组,差异有统计学意义(P<0.05)。结论:腹腔镜下肠粘连松解术治疗小儿粘连性肠梗阻的效果优于开腹肠粘连松解术,手术损伤小、术后可快速康复,并发症发生率、复发率更低。
Objective:To compare the effect of laparoscopic intestinal adhesion release and open intestinal adhesion release in the treatment of children with adhesive intestinal obstruction.Methods:A total of ninety children with adhesive intestinal obstruction in Yuncheng County Dinglichang Town Health Center from August 2021 to June 2023 were selected as the study objects,and were randomly divided into laparoscopic group and open group,with forty-five cases in each group.Laparoscopic intestinal adhesion release was performed in the laparoscopic group,and laparotomy was performed in the open group.The related indexes,clinical efficacy,postoperative complications and recurrence were compared between the two groups.Results:The operation time,postoperative waking time,bowel sound recovery time,gastrointestinal function recovery time,anal exhaust time,abdominal pain disappearance time and hospital stay time in laparoscopic group were shorter than those in open group,and the intraoperative blood loss was less than that in open group,the difference was statistically significant(P<0.05);the total effective rate of clinical efficacy of the laparoscopic group was higher than that of the open group,the difference was statistically significant(P=0.007);the total incidence of postoperative complications and recurrence rate in the laparoscopic group were lower than those in the open group,and the differences were statistically significant(P<0.05).Conclusion:Laparoscopic intestinal adhesiolysis is better than open intestinal adhesion release in the treatment of children with adhesive intestinal obstruction,with less surgical injury,rapid postoperative recovery,and lower complication rate and recurrence rate.
