摘要
目的 探讨儿童肥胖与糖尿病视网膜病变(DR)之间的因果关联。方法 采用孟德尔随机化(MR)的分析方法,以儿童肥胖作为暴露因素,以DR作为结局,通过IEU OpenGWASproject网站获取暴露和结局的全基因组关联研究(GWAS)数据,并提取相关的单核苷酸多态性(SNPs)作为工具变量(IVs)。儿童肥胖数据共包含样本量13 848例,SNPs为2 442 739个,DR数据共包含样本量190 594例,SNPs为16 380 347个。采用逆方差加权(IVW)法、加权中位数(WME)、加权众数(WM)法和MR-Egger回归进行MR分析,以OR值及其95%CI评价儿童肥胖与DR的因果关联,并进行异质性检验、多效性分析和敏感性分析。结果 本研究共纳入5个SNPs作为IVs, IVW结果显示儿童肥胖与DR具有正向因果关联(OR=1.143,95%CI:1.067~1.224,P<0.001),且暴露因素的IVs不存在水平多效性,异质性检验显示不存在异质性,“留一法”敏感性分析提示结果稳定。结论 儿童肥胖与DR存在因果关联,儿童肥胖会增加DR的发生风险。
Objective This study aimed to explore the causal association between childhood obesity and diabetic retinopathy(DR).Methods The Mendelian randomization(MR)analysis was used.Genome-wide association study(GWAS)data for the exposure(childhood obesity)and outcome(DR)were obtained from the IEU OpenGWAS project website.Relevant single nucleotide polymorphisms(SNPs)were extracted as instrumental variables(IVs).The childhood obesity dataset consisted of 13848 individuals with 2442739 SNPs,while the DR dataset consisted of 190594 individuals with 16380347 SNPs.MR analyses were conducted using the inverse variance weighted(IVW)method,weighted median estimator(WME),weighted mode-based estimator(WM),and MR-Egger regression.Odds ratios(OR)with 95%CI were used to evaluate the causal relationship between childhood obesity and DR.Heterogeneity testing,multiplicative analyses,and sensitivity analyses were performed.Results Five SNPs were included as instrumental variables.The IVW method showed a positive causal relationship between childhood obesity and DR(OR=1.143,95%CI:1.067-1.224,P<0.001).There was no evidence of horizontal pleiotropy in the instrumental variables for the exposure factor,and heterogeneity testing showed no significant heterogeneity.The"leave-one-out"sensitivity analysis indicated the stability of the results.Conclusion There is a causal association between childhood obesity and diabetic retinopathy,with childhood obesity increasing the risk of developing diabetic retinopathy.
