胸水细胞学标本SHOX2和RASSF1A基因甲基化预测肺腺癌的临床价值

摘要 目的探析胸水细胞学标本矮小同源盒基因2(SHOX2)和RAS相关区域家族1A(RASSF1A)基因甲基化对预测肺腺癌的临床价值。方法收集80例患者的胸水标本,其中40例为可疑肺腺癌(观察组),40例为良性病变(对照组)。用实时荧光定量聚合酶链式反应(RT-PCR)测定胸水细胞学标本SHOX2、RASSF1A基因甲基化情况。比较两组的SHOX2、RASSF1A检测值差异,Logistic回归分析法探析SHOX2、RASSF1A基因与肺腺癌病情的关系,绘制受试者工作特征(ROC)曲线分析SHOX2、RASSF1A在肺腺癌病情诊断预测中的应用价值。结果观察组的SHOX2、RASSF1A基因甲基化程度明显高于对照组;Logistic回归分析表明,SHOX2、RASSF1A基因甲基化高表达、强表达是肺腺癌的危险因素。ROC曲线分析得,SHOX2、RASSF1A基因甲基化联合预测肺腺癌病情结果的AUC(0.821,95%CI:0.727~0.915)最大,敏感度、特异性分别为87.5%、97.5%,并且SHOX2、RASSF1A甲基化检测结果联合液基细胞学诊断结果,有助于补充细胞学诊断上的漏诊。结论SHOX2、RASSF1A基因甲基化在肺腺癌中呈高表达,肺腺癌受检者的胸水细胞学标本SHOX2、RASSF1A基因甲基化水平高,可侧面说明患者病情恶化风险较高,可为肺腺癌的临床诊断及病情预测提供重要参考,同时也可作为细胞学病理诊断的补充,并且SHOX2、RASSF1A基因甲基化联合检测可作为可疑肺腺癌无创伤性辅助检查指标。 Objective Analysis of the clinical value of dwarf homeobox gene 2(SHOX2)and RAS related region family 1A(RASSF1A)gene methylation in predicting lung adenocarcinoma using pleural effusion cytology specimens.Methods Eighty pleural effusion samples were collected from patients,of which 40 were from suspected lung adenocarcinoma cases(observation group)and 40 from benign lesions(control group).The methylation status of SHOX2 and RASSF1A genes in cytology specimens of pleural effusion was determined using Real-Time Fluorescent Quantitative Polymerase Chain Reaction(RT-PCR).Differences in SHOX2 and RASSF1A detection values between the two groups were compared.Logistic regression analysis was used to explore the relationship between SHOX2 and RASSF1A genes and the condition of lung adenocarcinoma.Receiver Operating Characteristic(ROC)curves were drawn to analyze the application value of SHOX2 and RASSF1A in the diagnostic prediction of lung adenocarcinoma.Results In the observation group,the methylation levels of SHOX2 and RASSF1A genes were significantly higher than in the control group.Logistic regression analysis indicated that high expression of methylation in SHOX2 and RASSF1A genes is a risk factor for lung adenocarcinoma.ROC curve analysis showed that the combined prediction of lung adenocarcinoma based on SHOX2 and RASSF1A gene methylation had the highest AUC(0.821,95%CI:0.727 to 0.915),with a sensitivity of 87.5%and a specificity of 97.5%.Furthermore,combining the methylation test results of SHOX2 and RASSF1A with liquid-based cytology diagnosis helps to supplement and reduce the rate of missed diagnoses in cytological examination.Conclusion High expression of SHOX2 and RASSF1A gene methylation is observed in lung adenocarcinoma.Elevated levels of SHOX2 and RASSF1A gene methylation in the pleural effusion cytology specimens of individuals being screened for lung adenocarcinoma may indirectly indicate a higher risk of disease progression.These findings can provide important references for the clinical diagnosis and prognosis prediction of lung adenocarcinoma.They also serve as a supplement to cytopathological diagnosis.Furthermore,the combined detection of SHOX2 and RASSF1A gene methylation can be used as a non-invasive auxiliary diagnostic marker for suspected cases of lung adenocarcinoma.
