摘要
目的系统分析支气管抽吸物(包括支气管肺泡灌洗液和冲洗液)中矮小同源盒2(SHOX2)和Ras相关结构域家族蛋白1A(RASSF1A)DNA甲基化对肺癌的诊断效能。方法检索PubMed、Embase、The Cochrane Library、Web of Science、Clinical Trail、CNKI、万方数据库发表的关于SHOX2和RASSF1A甲基化对肺癌诊断价值的文献,检索日期截至2021年5月11日,进行数据提取和质量评价,使用Stata 16.0软件进行Meta分析。结果本研究共纳入20篇文献,中文文献8篇,英文文献12篇。Meta分析结果显示:SHOX2甲基化诊断肺癌合并后的灵敏度、特异度、阳性似然比、阴性似然比和曲线下面积(AUC)分别为0.73、0.90、7.50、0.30和0.86。RASSF1A甲基化诊断肺癌合并后的灵敏度、特异度、阳性似然比、阴性似然比和AUC分别为0.51、0.96、14.10、0.50和0.82。SHOX2和RASSF1A甲基化联合诊断肺癌合并后的灵敏度、特异度、阳性似然比、阴性似然比和AUC分别为0.78、0.87、5.80、0.26、0.90。结论支气管抽吸物中SHOX2和RASSF1A甲基化对肺癌的诊断价值较高,对CT检查可疑肺癌的患者,其可能是肺癌早期筛查的重要手段。
Objective To systematically analyze the diagnostic performance of short stature homeobox 2(SHOX2)and Ras association domain family protein 1A(RASSF1A)DNA methylation in bronchial aspirates(including bronchoalveolar lavage and lavage fluids)for lung cancer.Method The literatures published on PubMed,Embase,The Cochrane Library,Web of Science,Clinical Trail,CNKI,and Wanfang database on the value of SHOX2 and RASSF1A methylation in the diagnosis of lung cancer were searched.The retrieval date was up to May 11,2021,data extraction and quality evaluation were performed,and Meta-analysis was performed using Stata 16.0 software.Result A total of 20 literatures were included in this study,including 8 Chinese literatures and 12 English literatures.Meta-analysis results showed that the sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and area under the curve(AUC)of SHOX2methylation in the diagnosis of lung cancer were 0.73,0.90,7.50,0.30 and 0.86,respectively.The sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and AUC of RASSF1A methylation in the diagnosis of lung cancer were0.51,0.96,14.10,0.50 and 0.82,respectively.The combined sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and AUC of SHOX2 and RASSF1A methylation in the diagnosis of lung cancer were 0.78,0.87,5.80,0.26,and 0.90,respectively.Conclusion SHOX2 and RASSF1A methylation in bronchial aspirate has high diagnostic value for lung cancer,which may be an important method for early screening of lung cancer in patients with suspicious lung cancer detected by CT.
