摘要目的探究突发性耳聋合并耳鸣患者疾病不确定感与心理痛苦的相关性。方法选取2020年1月—2022年6月本院收治的145例突发性耳聋合并耳鸣患者为研究对象,对患者进行调查,分析疾病不确定感的影响因素及其与心理痛苦的相关性。结果不同文化程度、听力受损程度、耳鸣主观分级突发性耳聋合并耳鸣患者中文版疾病不确定感患者量表(mishel uncertainty in illness scale for adults,MUIS-A)得分比较,差异具有统计学意义(P<0.05)。137例突发性耳聋合并耳鸣患者DT得分为(6.20±1.45)分,其中74例(54.01%)处于中度痛苦,58例(42.34%)处于重度痛苦。低文化程度、听力受损程度严重、耳鸣主观分级较高、DT得分较高均是影响突发性耳聋合并耳鸣患者疾病不确定感的独立危险因素(P<0.05)。突发性耳聋合并耳鸣患者疾病不确定感(MUIS-A总分)与心理痛苦(DT总分)呈正相关(r=0.483,P<0.05)。结论心理痛苦对突发性耳聋合并耳鸣患者疾病不确定感具有重要影响,因此临床工作者可通过降低患者的心理痛苦来降低疾病不确定感。 Objective To explore correlation between disease uncertainty and psychological pain of patients with sudden deafness and tinnitus.Methods The paper treated patients with sudden deafness and tinnitus admitted to our hospital from January 2020 to June 2022 as study subjects,investigated and analyzed influencing factors of disease uncertainty and correlation with psychological pain.Results There was statistically significant difference in scores of the Chinese version of Mishel Uncertainty in Illness Scale for Adults(MUIS-A)among patients with sudden deafness and tinnitus with different levels of education,hearing impairment,and subjective grading of tinnitus(P<0.05).DT scores of 137 patients with sudden deafness and tinnitus were(6.20±1.45),74 cases(54.01%)of which were in moderate pain and 58(42.34%)in severe pain.Low educational level,severe hearing loss,high subjective grading of tinnitus,and high DT scores were independent risk factors affecting disease uncertainty of patients with sudden deafness and tinnitus(P<0.05).There was positive correlation between disease uncertainty(MUIS-A total scores)and psychological distress(DT total scores)of patients with sudden deafness and tinnitus(r=0.483,P<0.05).Conclusion Psychological pain can have significant effect on disease uncertainty of patients with sudden deafness and tinnitus.Therefore,clinical workers should reduce disease uncertainty by reducing psychological pain of patients.
