摘要
目的分析不同时间窗内鼓室注射地塞米松治疗突发性耳聋患者的效果。方法从我院选取2018年6月至2020年6月接收治疗的突发性耳聋患者188例为本次实验的调查研究对象,随机分为对照组、观察组,每组94例。188例患者入院均接受基础治疗,对照组患者入院第7日开始在鼓室内注射地塞米松注射液,观察组患者在入院当日接受鼓室内注射地塞米松治疗,对比临床治疗结果。结果两组患者临床治疗总有效率对比无统计学差异(P>0.05);治疗前两组患者平均听力阈值对比无统计学差异(P>0.05),经治疗平均听力阈值逐渐下降,治疗4 d后、8 d后、12 d后观察组平均听力阈值均低于对照组(P<0.05),统计学差异明显;观察组眩晕恢复天数(5.20±1.32)d短于对照组(6.71±1.30)d(P<0.05),统计学差异明显;观察组耳鸣恢复天数(3.16±1.10)d短于对照组(5.23±1.23)d(P<0.05),统计学差异明显;观察组听力恢复天数(8.11±2.23)d短于对照组(9.51±2.25)d(P<0.05),统计学差异明显。结论突发性耳聋患者早期给予鼓室注射地塞米松治疗,可有效提高平均听力阈值,缩减眩晕、耳鸣、听力等恢复天数,减轻治疗初期全身副反应,促进康复,适用于临床治疗大量的推广和使用。
Objective To analyze the effect of intratympanic dexamethasone injection in different time windows on patients with sudden deafness.Methods One hundred and eighty-eight patients with sudden deafness who received treatment from June 2018 to June 2020 in our hospital were selected as the research objects of this experiment.They were randomly divided into control group and observation group,with 94 cases in each group.All 188 patients received basic treatment.Dexamethasone was injected into the tympanic cavity on the 7th day of admission in the control group,while dexamethasone was injected into the tympanic cavity on the day of admission in the observation group.Compare the clinical treatment results.Results There was no significant difference in the total effective rate between the two groups (P>0.05).There was no significant difference in the average hearing threshold between the two groups before treatment (P>0.05).After treatment,the average hearing threshold gradually decreased,and the average hearing threshold of the observation group was lower than that of the control group after 4 days,8 days and 12 days (P<0.05).The recovery time of vertigo in the observation group (5.20±1.32) d was shorter than that in the control group (6.71±1.30) d (P<0.05).The recovery time of tinnitus in the observation group (3.16±1.10) d was shorter than that in the control group (5.23±1.23) d (P<0.05).The hearing recovery days in the observation group (8.11±2.23) d were shorter than those in the control group (9.51±2.25) d (P<0.05).Conclusion Early treatment of sudden deafness with dexamethasone injection in tympanic cavity can effectively improve the average hearing threshold,reduce the recovery days of vertigo,tinnitus and hearing,reduce the systemic side effects at the initial stage of treatment,and promote rehabilitation,which is suitable for a large number of clinical treatment and use.
