摘要
目的探讨瘢痕内注射曲安奈德联合CO_(2)点阵激光治疗增生性瘢痕的效果及对皮损情况、瘢痕血流灌注的影响。方法选取2019年2月至2022年2月收治的100例增生性瘢痕患者作为研究对象,随机将其分为对照组和观察组,各50例。对照组采用CO_(2)点阵激光治疗,观察组在对照组基础上联合瘢痕内注射曲安奈德治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的改良温哥华瘢痕量表(mVSS)评分、瘢痕厚度及瘢痕血流灌注量均低于对照组(P<0.05)。治疗后,观察组的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论瘢痕内注射曲安奈德联合CO_(2)点阵激光治疗增生性瘢痕可提高疗效,促进皮损情况改善,减少瘢痕血流灌注量,也能减轻炎性反应。
Objective To investigate the effect of intra-scar injection of triamcinolone acetonide combined with CO_(2)fractional laser in the treatment of hypertrophic scar and its influences on skin lesions and scar blood perfusion.Methods A total of 100 patients with hypertrophic scar admitted from February 2019 to February 2022 were selected as the research objects and randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with CO_(2)fractional laser,and the observation group was treated with intra-scar injection of triamcinolone acetonide on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the modified Vancouver Scar Scale(mVSS)score,scar thickness and scar blood perfusion in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Intra-scar injection of triamcinolone acetonide combined with CO_(2)fractional laser in the treatment of hypertrophic scar can improve the curative effect,promote the improvement of skin lesions,and reduce the amount of scar blood perfusion and the inflammatory response.
