重复经颅磁刺激治疗脑卒中后失语症的临床研究 被引量:2

Clinical Study on Repeated Transcranial Magnetic Stimulation in the Treatment of Aphasia after Stroke

摘要 目的:观察重复经颅磁刺激治疗脑卒中后失语症的效果。方法:选取2019年1月—2020年12月于南昌大学第三附属医院收治的脑卒中后失语症患者90例为研究观察对象,按照随机数字表方法将其分为A组(30例)、B组(30例)、C组(30例)。A组采用常规言语训练干预,B组采用经颅磁刺激干预,C组采用经颅磁刺激+常规言语训练干预。比较三组治疗前与治疗2、4周后汉语失语成套测验、失语指数(AQ)评分和功能性核磁共振(fMRI)检查结果。结果:治疗前三组谈话、理解、复述、命名、阅读、写病情评分比较,差异均无统计学意义(P>0.05);治疗2、4周后,C组上述评分均高于A组、B组,差异均有统计学意义(P<0.05)。治疗前,三组AQ评分比较,差异均无统计学意义(P>0.05);治疗2、4周后,C组AQ评分均高于A组、B组,差异均有统计学意义(P<0.05)。治疗前,三组胆碱(Cho)/血肌酐(Scr)、N-乙酰天冬氨酸(NAA)/Scr、Lac/Scr比较,差异均无统计学意义(P>0.05);治疗2、4周后,C组Cho/Scr、乳酸(Lac)/Scr均低于A组、B组,C组NAA/Scr高于A组、B组,差异均有统计学意义(P<0.05)。结论:脑卒中后失语症患者采用重复经颅磁刺激治疗,能够调控神经活动,有效改善言语功能及失语症状。 Objective:To observe the effect of repeated transcranial magnetic stimulation in the treatment of aphasia after stroke.Method:A total of 90 patients with aphasia after stroke admitted to the Third Affiliated Hospital of Nanchang University from January 2019 to December 2020 were selected as the research objects.According to the random number table method,they were divided into group A(30 cases),group B(30 cases)and group C(30 cases).Group A received conventional speech training intervention,group B received transcranial magnetic stimulation intervention,and group C received transcranial magnetic stimulation+conventional speech training intervention.The scores of aphasia battery in Chinese,aphasia quotient(AQ),and the results of functional magnetic resonance imaging(fMRI)were compared among the three groups before treatment and 2,4 weeks after treatment.Result:Before treatment,there were no significant differences in the scores of talking,understanding,retelling,naming,reading and writing about illness among the three groups(P>0.05);after 2 and 4 weeks of treatment,the above scores in group C were higher than those in group A and group B,the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in the scores of AQ among the three groups(P>0.05).after 2 and 4 weeks of treatment,the scores of AQ in group C were higher than that of group A and group B,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in choline(Cho)/serum creatinine(Scr),N-acetyl aspartate(NAA)/Scr and lactic acid(Lac)/Scr among the three groups(P>0.05);after 2 and 4 weeks of treatment,Cho/Scr and Lac/Scr in group C were lower than those in group A and group B,while NAA/Scr in group C was higher than that in group A and group B,the differences were statistically significant(P<0.05).Conclusion:Repeated transcranial magnetic stimulation can regulate neural activity and effectively improve speech function and aphasia symptoms in patients with aphasia after stroke.
