摘要
目的探讨血清CA125、NGAL、HE4在子宫内膜癌患者中的表达及预后评估价值。方法选取本院2019年1月至2020年6月收治的66例子宫内膜癌患者作为研究分析对象,对比不同病理特征子宫内膜癌患者血清CA125、NGAL、HE4水平差异,并连续随访3年,根据患者预后情况分为两组,其中预后良好组42例,预后不良组24例,对比两组患者血清CA125、NGAL、HE4水平差异,并分析血清CA125、NGAL、HE4的诊断效能。结果病理分期为III~IV期患者血清各项水平均高于I~II期患者,肌层浸润深度≥1/2肌层患者血清各项水平均高于肌层浸润深度<1/2子宫内膜癌患者(P<0.05),淋巴结转移、脉管浸润患者血清各项水平均高于无淋巴结转移、无脉管浸润患者(P<0.05);不同病理类型患者血清各项水平比较大差异(P>0.05)。预后不良组子宫内膜癌患者血清各项水平均高于预后良好组(P<0.05)。ROC曲线分析显示,血清CA125、NGAL、HE4联合检测预测子宫内膜癌不良预后的AUC(95%CI)为0.976(0.948~0.999),灵敏度95.8%与特异度96.9%两者均明显高于各指标。结论血清CA125、NGAL、HE4在子宫内膜癌患者中呈高水平表达,且联合检测可提高子宫内膜癌患者不良预后的诊断效能。
Objective To investigate the expression and prognostic value of serum CA125,NGAL and HE4 in patients with endometrial carcinoma.Methods 66 cases of endometrial cancer patients admitted to our hospital from January 2019 to June 2020 were selected as research analysis objects.The differences in serum CA125,NGAL and HE4 levels of endometrial cancer patients with different pathological characteristics were compared,and the patients were followed up for 3 years.They were divided into two groups according to the prognosis,among which 42 cases had a good prognosis.There were 24 patients in the poor prognosis group.The difference of serum CA125,NGAL and HE4 levels between the two groups was compared,and the diagnostic efficacy of serum CA125,NGAL and HE4 was analyzed.Results The serum levels in patients with stage III to IV were higher than those in patients with stage I to II,and the serum levels in patients with≥1/2 myoinfiltration depth were higher than those in endometrial cancer patients with<1/2 myoinfiltration depth P<0.05.The serum levels of patients with lymph node metastasis and vascular invasion were higher than those without lymph node metastasis and vascular invasion(P<0.05).The serum levels of different pathological types were compared(P>0.05).The serum levels of patients with endometrial cancer in poor prognosis group were higher than those in good prognosis group(P<0.05).ROC curve analysis showed that the AUC(95%CI)of serum CA125,NGAL and HE4 combined detection to predict the adverse prognosis of endometrial cancer was 0.976(0.948~0.999),and the sensitivity of 95.8%and specificity of 96.9%were significantly higher than each index.Conclusion Serum CA125,NGAL and HE4 are highly expressed in patients with endometrial cancer,and combined detection can improve the diagnostic efficiency of poor prognosis in patients with endometrial cancer.
