摘要
目的探讨甲泼尼龙琥珀酸钠联合利多卡因耳后注射治疗高频下降型突发性耳聋(sudden deafness,SD)伴耳鸣的临床疗效及对血液流变学、血管内皮损伤的影响。方法选取2021年6月—2023年6月吉林大学第一医院梅河医院收治的90例高频下降型SD伴耳鸣患者为研究对象,采用随机数表法分为研究组和对照组,各45例,对照组给予甲泼尼龙琥珀酸钠联合利多卡因常规静脉给药治疗,研究组则给予甲泼尼龙琥珀酸钠联合利多卡因耳后骨膜下注射治疗,比较两组患者临床疗效、血清一氧化氮(nitric oxide,NO)、内皮素-1(endothelin-1,ET-1)及血液流变学指标变化。结果治疗1个疗程后,研究组总有效率明显高于对照组(91.11%vs 73.33%),差异有统计学意义(χ^(2)=4.865,P<0.05)。研究组血浆黏度、血细胞比容、ET-1明显低于对照组,红细胞变形指数、NO明显高于对照组,差异有统计学意义(P<0.05)。结论对高频下降型SD伴耳鸣患者采用甲泼尼龙琥珀酸钠联合利多卡因耳后骨膜下注射能显著提高临床疗效,并能进一步降低血液黏度,促进血管内皮损伤的修复,改善内耳微循环。
Objective To investigate the clinical efficacy and effect on blood rheology and vascular endothelial dam-age of methylprednisolone sodium succinate combined with lidocaine postauricular injection in the treatment of high-frequency descending sudden deafness(SD)with tinnitus.Methods A total of 90 patients with high frequency de-scending SD with tinnitus admitted to Meihe Hospital,the First Hospital of Jilin University from June 2021 to June 2023 were selected as the study subjects,and were divided into study group and control group by random number table method,45 cases in each group.The control group was given routine intravenous administration of methylpred-nisolone sodium succinate combined with lidocaine,while the study group was treated with methylprednisolone so-dium succinate combined with lidocaine retroaural periosteal injection.Clinical effects and endothelin-1(NO),endo-thelin-1(ET-1)and hemorheology indicators in the two groups were compared.Results After one course of treatment,the total effective rate of the study group was significantly higher than that of the control group(91.11%vs 73.33%),and the difference was statistically significant(χ^(2)=4.865,P<0.05).The plasma viscosity,hematocrit and ET-1 of the study group were significantly lower than those of the control group,while the erythrocyte deformation index and NO were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Con-clusion The use of methylprednisolone sodium succinate combined with lidocaine postauricular subperiosteal injec-tion in patients with high-frequency descending SD with tinnitus can significantly improve the clinical efficacy and further reduce blood viscosity,promote the repair of vascular endothelial damage,and improve the microcirculation of the inner ear.
