微动力负压技术在促进糖尿病足溃疡患者创面愈合及降低创面细菌感染风险中的应用价值

The effect of microdynamic negative pressure technique on promoting wound healing and reducing the risk of bacterial infection in diabetic foot ulcer patients

摘要 目的探究微动力负压技术应用于糖尿病足溃疡患者的效果及对创面愈合及降低创面细菌感染的影响。方法选取黄河水利委员会黄河中心医院伤口中心2020年11月至2022年11月期间收治的98例糖尿病足溃疡患者,根据数字表法随机分组。两组均给予抗菌药物治疗,对照组49例给予泡沫塑料类敷料,观察组49例给予微动力负压技术干预。2周后,对比两组患者溃疡面愈合情况(创面愈合率、溃疡完全愈合时间、换药次数、随访1个月溃疡复发率)、溃疡面渗液指标[血管内皮生长因子(vascular endothelial growth factor,VEGF)、基质金属蛋白酶(matrix metalloproteinase-2,MMP-2)和基质金属蛋白酶组织抑制剂(tissue inhibitor of matrix metalloproteinases 1,TIMP-1)水平]、创面细菌培养阳性情况及不良反应发生率。结果两组患者创面愈合率、溃疡复发率、不良反应发生率差异无统计学意义(P>0.05)。干预后,观察组患者溃疡完全愈合时间、换药次数、细菌培养阳性率和MMP-2水平均低于对照组,VEGF和TIMP-1水平高于对照组,差异具有统计学意义(P<0.05)。结论与泡沫塑料类敷料相比,利用微动力负压技术对糖尿病足溃疡患者进行干预可有效调节渗液生长因子水平,降低细菌阳性率,促进创面快速愈合,安全性高。 Objective To explore the effect of microdynamic negative pressure technique on diabetic foot ulcer patients and its effect on wound healing and reducing bacterial infection.Methods A total of 98 patients with diabetic foot ulcer from November 2020 to November 2022 in Wound Center of Yellow River Central Hospital were selected and grouped according to random number table method.Both groups were treated with antibiotics,49 cases in the control group were treated with foam plastic dressing,and 49 cases in the observation group were treated with microdynamic negative pressure technology intervention.Two weeks later,the healing situation of ulcer surface in the two groups including wound healing rate,complete healing time of ulcer,dressing change times,and 1-month follow-up ulcer recurrence rate was compared.The effusion indexes of ulcer surface including vascular endothelial growth factor(VEGF),matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase tissue inhibitor-1(TIMP-1)were further compared.Positive bacterial culture of wound surface and incidence of adverse reactions between two groups were also compared.Results After 1 month of follow-up,the wound healing rate,ulcer recurrence rate and incidence of adverse reactions were not statistically significant between the two groups(P>0.05).After intervention,the ulcer complete healing time,dressing change times,bacterial culture positive rate and MMP-2 level in the observation group were significantly lower than those in the control group,while the VEGF and TIMP-1 levels in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Compared with foam plastic dressings,microdynamic negative pressure technique can effectively regulate the level of growth factor in effusion,reduce the positive rate of bacteria,and promote rapid wound healing with high safety in patients with diabetic foot ulcer.
