摘要
目的比较双切口双钢板内固定与单侧锁定钢板内固定对SchatzkerⅤ/Ⅵ型胫骨平台骨折患者膝关节内翻角、后倾角的影响。方法选取2020年8月至2022年1月至福建省建瓯市立医院就诊的100例SchatzkerⅤ/Ⅵ型胫骨平台骨折患者作为研究对象,根据手术方式分为观察组(n=50)和对照组(n=50),观察组采用双切口双钢板内固定治疗,对照组给予单侧锁定钢板内固定治疗。比较两组患者的疗效和手术前后膝关节内翻角、后倾角的变化情况。结果观察组的临床疗效优良率为84.00%,高于对照组的66.00%,差异有统计学意义(P<0.05)。术前,两组患者的膝关节内翻角、后倾角比较,差异无统计学意义(P>0.05);术后12个月,观察组的内翻角、后倾角均小于对照组,差异有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论针对SchatzkerⅤ/Ⅵ型胫骨平台骨折,双切口双钢板内固定的临床疗效优于单侧锁定钢板内固定,更具稳定性,安全性更高,值得临床应用。
Objective To compare the effect of double-incision double-plate internal fixation and unilateral locking plate internal fixation on the varus angle and posterior slope of knee joint in patients with SchatzkerⅤ/Ⅵtype tibial plateau fractures.Methods A total of 100 patients with SchatzkerⅤ/Ⅵtype tibial plateau fractures treated in Jian'ou Municipal Hospital,Fujian Province from August 2020 to January 2022 were selected as the research objects.According to the surgical method,they were divided into observation group(n=50)and control group(n=50).The observation group was treated with double-incision double-plate internal fixation,while the control group was treated with unilateral locking plate internal fixation.The curative effect and the changes of knee varus angle and posterior slope angle before and after operation were compared between the two groups.Results The excellent and good rate of clinical efficacy in the observation group was 84.00%,which was higher than 66.00%in the control group,and the difference was statistically significant(P<0.05).Before operation,there was no significant difference in knee varus angle and posterior slope angle between the two groups(P>0.05).At 12 months after operation,the varus angle and posterior slope angle of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For SchatzkerⅤ/Ⅵtype tibial plateau fractures,double-incision double-plate internal fixation has better clinical efficacy than unilateral locking plate internal fixation,which is more stable and safer,and is worthy of clinical application.
