摘要
目的:探究锁定钢板内固定并植骨治疗复杂胫骨平台骨折(CTPF)的效果。方法:选取2021年8月—2023年8月贵州省建筑医院收治的94例CTPF患者。随机将其分为对照组和观察组,各47例。对照组给予锁定钢板内固定治疗,观察组给予锁定钢板内固定并植骨治疗。比较两组围手术期指标,术后即刻、术后3个月胫骨平台内翻角与后倾角,术前、术后3个月膝关节功能、生活质量及并发症。结果:观察组术中出血量多于对照组,手术时间长于对照组,开始负重时间和骨折愈合时间均早于对照组,差异有统计学意义(P<0.05)。术后3个月,两组胫骨平台内翻角、胫骨平台后倾角、美国特种外科医院膝关节功能(HSS)评分均升高,观察组胫骨平台内翻角、胫骨平台后倾角均小于对照组,HSS评分高于对照组,差异有统计学意义(P<0.05)。术后3个月,两组心理功能、躯体功能、社会功能、物质生活状态评分均升高,观察组心理功能、躯体功能、社会功能、物质生活状态评分均高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:锁定钢板内固定并植骨可维持CTPF患者的膝关节平稳度,改善膝关节功能,提高生活质量,且无严重并发症。
Objective:To explore the effect of locking steel plate internal fixation and bone grafting in the treatment of complex tibial plateau fractures(CTPF).Method:A total of 94 CTPF patients treated in Guizhou Construction Hospital from August 2021 to August 2023 were selected.They were randomly divided into control group and observation group,47 cases in each group.The control group was treated with locking plate internal fixation,and the observation group was treated with locking plate internal fixation and bone grafting.Perioperative indexes,varus angle and posterior inclination angle of tibial plateau immediately after surgery and 3 months after surgery,knee function,quality of life before and 3 months after surgery and complications of the two groups were compared.Result:The intraoperative blood loss volume in the observation group was more than that in the control group,the operation time was longer than that in the control group,and the time to start weight bearing and fracture healing were earlier than those in the control group,the differences were statistically significant(P<0.05).At 3 months after surgery,the varus angle and posterior inclination of tibial plateau and the Hospital for Special Surgery knee function score(HSS)scores in both groups were increased,and the varus angle and posterior inclination of tibial plateau in the observation group were less than those in the control group,and the HSS score was higher than that in the control group,the differences were statistically significant(P<0.05).Three months after surgery,the scores of psychological function,physical function,social function and material life status of both groups were increased,and the scores of psychological function,physical function,social function and material life status of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Lock plate internal fixation and bone grafting can maintain the knee smoothness,improve the knee function,improve the quality of life in patients with CTPF,without serious complications.
