摘要
目的分析2013—2021年广东省云浮市恙虫病流行病学特征,为制定科学有效的防控措施提供参考。方法从中国疾病预防控制信息系统传染病报告信息管理系统中导出2013年1月1日至2021年12月31日恙虫病病例监测数据资料,采用描述流行病学方法分析人群等分布特征及发病率指标,采用集中度法和圆形分布分析季节特征。结果2013—2021年云浮市累计报告恙虫病病例3293例、死亡1例,报告发病率8.55/10万~19.09/10万,年均报告发病率为14.82/10万,发病率呈缓慢上升趋势(趋势χ^(2)=26.521,P<0.05);集中度(M)值为0.43,提示恙虫病发病有一定的季节特征;圆形分布法显示,每年的4月中旬至9月上旬为流行期,属于夏秋季节型;报告病例以云城区(937例)和云安区(902例)较多、占总数的55.85%;男女性别比1∶1.18,40岁及以上的报告病例数占总数的85.91%,职业以农民病例为主、占总数的82.02%。结论云浮市恙虫病发病率呈波动上升趋势,建议在每年的4—9月重点加强云城区和云安区、40岁以上农民的健康宣传教育和监测预警工作。
Objective To analyze the epidemiological characteristics of tsutsugamushi disease in Yunfu city of Guangdong province from 2013 to 2021,and provide reference for developing scientific and effective prevention and control measures.Methods The surveillance data of the cases with tsutsugamushi disease from January 1,2013 to December 31,2021 was derived from Infectious Disease Reporting Information Management System of the Chinese Disease Prevention and Control Information System.The indicators of distribution characteristics and incidence in the population were analyzed with descriptive epidemiological methods,and the seasonal characteristics were analyzed with concentration method and circular distribution.Results There were 3293 cases with tsutsugamushi disease and 1 case death reported in Yunfu city from 2013 to 2021,with the reported incidence rates from 8.55/100000 to 19.09/100000,and the annual average reported incidence rate of 14.82/100000.The incidence rates showed slow increasing trend(trendχ^(2)=26.521,P<0.05).The concentration ratio(M)was 0.43,that indicated a certain seasonal characteristic of the incidence of tsutsugamushi disease.The circular distribution method suggests that the epidemic period was from mid-April to early September each year,and was the summer and autumn seasonal type.The reported cases were mainly in Yuncheng district(937 cases)and Yun'an district(902 cases),accounting for 55.85%of the total number of the cases.The ratio of the male to the female was 1∶1.18.The reported cases aged 40 years and above accounted for 85.91%.The occupation of the cases was farmer,accounting for 82.02%.Conclusions The incidence rates of tsutsugamushi disease show fluctuating upward trend in Yunfu city.It is recommended to strengthen the health education,monitoring and early warning in farmers aged 40 and above in Yuncheng district and Yun'an district during the April to September each year.
